Deliver peer support services to people transitioning from psychiatric or substance abuse from the facility into their own communities through individual and group peer support meetings
Engage referred individuals to form trust-based relationships that fosters hope for positive change and personal empowerment.
Continue with individuals after discharge to identify and engage with a range of community-based supports.
Complete detailed written record of activities.
Report to and meet with supervisor on regular basis and attend required training sessions.
Maintain personal wellness.
HS Diploma or equivalent. Flexible schedule with ability to work evenings and/or weekends as needed. Continual internet and email access.
Clean, valid driver's license with own vehicle.
Lived experience with the mental health system is HIGHLY preferred.
