Bi-Lingual Licensed Clinical Social Worker (LCSW)

Clinical Social Worker (LCSW) will work collaboratively with the CMO and Behavioral Health Providers to co-lead the mental/behavioral health programming. The LCSW will implement quality improvement initiatives for mental/behavioral health and social services. LCSW will supervise any master’s level social worker, mental health care managers and social services case managers. LCSW will also provide clients with psychotherapy (individual, couples and family therapy), care and case management services, crisis counseling, and outreach services in the community to eligible clients. LCSW will create topics for educational support groups and design therapeutic group programs for clients within the community. LCSW will identify client’s individual needs by creating a psychosocial, rehabilitative and medical services case plan.

MINIMUM QUALIFICATIONS:

  • Licensed Clinical Social Worker: Must pass the Licensed Clinical Social Worker exam for the State of Louisiana.
  • Experience working in a health care facility and a social services agency.
  • Experience in providing individual, couples and family therapy. Experience in running group therapy.
  • Experience in conducting psychotherapy, cognitive behavioral therapy and problem solving therapy
  • Familiarity with governmental programs such as Social Security, Medicaid, Medicare, Social Security, etc.
  • Ability to use various standardized word processing and spreadsheet software required; working knowledge of Microsoft Word, and Excel.
  • Excellent oral and written communication skills.
  • Ability to work in a culturally diverse, patient-centered, holistic atmosphere.
  • Familiar with operation of electronic office machines (phones, copier, fax).

KEY RESPONSIBILITIES AND PERFORMANCE STANDARDS

  • Mental and Behavioral Health-Psychotherapy: (65%)
  • Completes in-depth psychosocial assessments and formulates an appropriate treatment plan including clinical impressions.
  • Responds, evaluates and intervenes in crisis situations with clients, by providing crisis and brief counseling.
  • Manages a psychotherapy caseload of 40 active clients, seen weekly, bi-weekly or monthly.
  • All clients are also active care management clients- initiating PHQ-9 (depression clients) at least 1x monthly, GAD-7 at least monthly (anxiety clients), monitor their psychotropic medications based on self- report of symptoms and side effects, track evidence based depression/anxiety scores to monitor treatment outcomes.
  • Suggest medication titration, reduction or complete medication evaluation to primary care physicians or psychiatrist for behavioral health clients.
  • Conduct problem solving and cognitive behavioral therapy techniques to conduct approximately 8-12 week therapeutic session with clients.
  • Provides individual, family, couples and group psychotherapy for child/adolescent psychiatric patients and their families.
  • Provides individual, family, couples and group psychotherapy for adult psychiatric patients and their families.
  • Conduct vocational, educational, substance abuse counseling.
  • Reinforce communication with physicians regarding patients’ needs such as relating their psychosocial stressors and mental health issues in order to aid medical holistic treatment of the patients.
  • Coordinate patient admission to drug detoxification programs
  • Accurately and appropriately document mental health communications, observations, interventions and referrals.
  • Supervision of Social Services Support Staff (25%)
  • Tracks all social service referrals from clinic personnel and assign to care/case manager as appropriate
  • Supervises mental/behavioral health care manager 1x weekly -1 hour and is available for consultation continuously throughout the day in person or through email.
  • Supervise case manager for tasks including but not limited to:
  • Addressing psychosocial needs of clients by referring them to existing community programs and social support organizations, such as shelters, substance abuse programs, job assistance programs, nursing facilities etc.
  • Providing ongoing case management services and reassess client needs including ongoing eligibility for services; tracks client progress and well being, evaluate case plan effectiveness and effectively manages resources, prevents conflicting case plans and duplication of services.
  • Completion of governmental applications such as Medicaid, Medicare, LaChip, FEMA, Road Home, etc.
  • Completion of pharmacy assistance applications
  • Supervise GSW 1x weekly -1 hour to discuss case management and psychotherapy issues regarding GSW’s clients.
  • Monitors social work/therapy notes from care manager, case managers, GSW, primary care physicians, and psychiatrist.
  • Quality Improvement (5%)
  • Work collaboratively with the psychiatrist to develop and implement policies and procedures for mental/behavioral health programming
  • Performance reporting on the following services:
  • Mental/behavioral health care management
  • Social services case management
  • Additional tasks as needed (5%)
  • Cross trains with other clinical staff to assist so as to ensure highest level of quality care to NOELA CHC patients.
  • Exhibits a willingness to learn new tasks.
  • Asks questions before beginning an unfamiliar assignment.
  • Licensed Clinical Worker will attend appropriate trainings to obtain continuing education units to maintain licensure yearly.
