► Complete a comprehensive review and assessment of patients’ medical history, current medications, psycho-social well-being and social determinants of health
► Document progress notes that support comprehensive capture of all active diagnoses
► Provide incomplete preventive care services as determined by HEDIS or Medicare 5 Star requirements
► Perform point-of-care lab and diagnostic testing, such as Hemoglobin A1C, monofilament, nephropathy screening and diabetic retinal exams
► Follow-up on abnormal results and coordinate ongoing care with PCP
► Write prescriptions with the support and oversight of a collaborating physician
► Help at-risk patients transition from the hospital and other facilities to their own home through in-person assessments, post discharge medication reconciliation, referrals and care coordination with the PCP
► Board certified as an Advanced Nurse Practitioner in the specialty area of family medicine
► Hold an active, unrestricted state Nurse Practitioner license, as well as DEA and NPI number
► Have a current provider card in Basic Life Support (BLS)
► Comfortable walking up flights of stairs while carrying up to 50 lbs. of equipment, as some residences may not have elevators
► At least 3 years of clinical experience preferably in home health, palliative care and/or hospice settings

Nurse Practitioners are only traveling within 1-2 zip codes in any given day, you will never travel more than 125 round trip on a given day – and of course mileage is reimbursed!
• Annual Comprehensive Exams: Annual physical, review & documentation of patient’s blood pressure, BMI, and other vital stats; medical history, active diagnosis review, medication review & education, and various point-of-care tests. • Quality Visits: - Comprehensive Diabetes Care: A1C, nephrology screen, Diabetic Retinal Exam, etc. - Schizophrenic screening for diabetes - “Mothers of Molina” – Postnatal depression screening & counseling on immunizations and follow-up appointments for new mothers - Well Child Exams – including immunizations. Well Child Exams are performed at our “Pop-Up Clinics,” which are hosted at various community organizations like YMCA or Boys & Girls Clubs, and faith-based organizations.
EXEMPT salaried position, no overtime, 40 hours a week:
4x10 schedule or a 5/8 schedule
• 4x10: one mandatory Saturday per month (we ask providers to start their day no later than 8:00am for this shift as 10 hours later, especially in the winter months, it gets dark early, and our providers safety is our top priority.)
Full schedule: 8 visits/day • 5x8: one mandatory Saturday per month, one late day per week/extended days (hours) during seasons with more daylight Full schedule: 6 visits/day 4. Passion to work with at-risk populations; and understands the conditions they will be working in…and still love it.
Our Nurse Practitioners are not rushed. They spend 45 minutes to 1 hour with each member on their visits – REALLY!!
This provides the opportunity for comprehensive documentation of member conditions, diagnoses and needs, and for delivering advanced point-of-care testing that creates the space to educate members on their health conditions.
