TITLE: RN Patient Access Navigator
SUPERVISION: Director of Access
POSITIONS SUPERVISED: None
PRIMARY FUNCTION: Serve as a liaison in a consultant role to hospitals, consult at patient homes, discharge planners, nursing homes, physician offices, and other Healthcare facilities regarding Hospice and Palliative Care. Generate referrals and perform admissions as needed. Consulting role is focused in the Winston-Salem area.
- Education and/or Experience: A Bachelor’s degree in nursing is preferred. At least 3 years of experience in hospice or home health preferred. At least 2 years experience in marketing, customer service, or liaison role a plus.
- Language/Computer Skills: Ability to read and comprehend and write simple instructions, short correspondence, and memos. Ability to effectively present information in one-on-one and small group situations to patients/families/caregivers, employees, volunteers, referral sources, physicians, and others as needed. Must have strong communication skills. Must be proficient in Microsoft Office applications, email, and internet.
- Math Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
- Reasoning Ability: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Must demonstrate excellence in critical thinking and prioritizing. Ability to be flexible, well organized, handle pressure, manage a variety of tasks, and tend to detail.
- Interpersonal and Organizational Skills: Must possess strong interpersonal skills, and ability to relate well with a variety of people. Must possess excellent organizational and planning skills. Must have a commitment to and understanding of the Hospice Concept.
- Certificates, Licenses, Registrations: Must possess a valid Driver’s License and current auto insurance. Must possess active registered compact nursing license in North Carolina or Virginia if applicable. Maintains Healthcare Provider Level CPR Certification.
- Continuing Education Requirements: Expected to participate in appropriate continuing education to meet job requirements. In addition, expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities, and to attend mandatory educational programs. Must have at least 12 hours of annual education hours.
- Performance Improvement and Compliance Requirements: Expected to participate in the Quality Assessment Improvement Program (QAPI). Expected to comply with Federal and State regulations governing hospice programs, Nurse Practice Act, Board of Pharmacy requirements, accrediting standards, MVHPC policies and procedures and to adhere to the organization’s standards of conduct as a condition of continued employment.
- Develop a Hospice presence in the area hospitals, nursing homes, and adult care homes attending rounds and conferences when possible, conferring with providers and speaking at meetings.
- Serve as a consultant to medical professionals concerning the hospice program, palliative care, pain management, specialty programs and about Hospice and palliative care reimbursement.
- Act as a liaison for Hospice patients admitted to the hospital. Ensure that the Hospice plan of care is adhered to during the in-patient admissions. Help determine if the inpatient admission is related to the terminal illness. Help to ensure a smooth discharge process.
- Work with physicians, nurses, social workers, and other medical professionals to ensure palliative care is administered to hospice patients.
- Inform appropriate hospital patients and their families about the Hospice philosophy, care and services.
- Respond to referrals/consults promptly. Schedule a meeting with the patient and family and if necessary, conduct a full assessment/inpatient admission according to established protocol.
- Work in conjunction with hospital case managers regarding current patients, new referrals, and provides consultation and follow up as requested.
- Confer frequently with area Hospice programs reinforcing collaborative relationships.
- Participate in community education and health fairs.
- Assist in the North Carolina Nurses Association Continuing Education activity planning process as a Content Expert, Target Audience Representative or Nurse Planner and serve as a member of the approved provider unit as assigned.
- Perform other duties as assigned by the supervisor.
Regular visits to physician practices, hospitals nursing facilities, patient’s home, nursing home, or rest home with regular exposure to hazardous conditions including blood, bodily fluids, chemotherapy aspects, and communicable diseases. There is limited exposure to hazardous cleaning solutions. Extensive time spent traveling by car to work location.
