Midtown Health Center, a Federally Qualified Health Center serving Northeast Nebraska with focus on improving access to complete care for the whole family is looking for an energetic and compassionate individual to immediately fill the Case Manager position in our organization.

Experience in behavioral health case manager is a plus.

Proficiency in English and Spanish is desirable and wage differential will be offered to successful bilingual candidate.

General Summary

Provides case management services that help Midtown Health Center patients gain access to needed medical, behavioral health, and other services. Position involves assessment of the patient to determine needs; referral and related activities to help the patient obtain needed services; and monitoring, follow-up, and evaluation activities. The position provides patient education and support as appropriate. The goal of the case manager is to work with patients as referred/assigned to gain self-sufficiency and maximized health through self-management.

Essential Duties and Responsibilities

  1. Maintains an active caseload providing interventions as needed within area of expertise and limits of credentials.
  2. Identifies and addresses patients’ risk factors and/or obstacles to care.
  3. Ensures patients are screened for medical, behavioral health, social, and other health related needs.
  4. Assists in the development, implementation, and revision of individual treatment plans; assures that services provided are specified in the treatment plan and monitors progress toward treatment goals.
  5. Assists providers in patient care as directed, including ensuring patients have access appropriate access to medication.
  6. Educates patients/families/caregivers about the case management process and evaluates their understanding of the process.
  7. Connects patients to services and resources to meet their needs and goals of treatment plans.
  8. Communicates the care preferences of patients, serving as their advocate, and verifying that inventions meet patients’ needs and goals of treatment.
  9. Works with patients on a day-to-day basis using professional judgment and discretion to implement the team-determined treatment plan with the goal of patients gaining self-management skills and self-sufficiency.
  10. Works towards the improvement of established performance indicators.
  11. Documents patients’ case management plans and on-going activities in accordance with applicable standards and regulations.
  12. Serves as a liaison between patients and providers to resolve care concerns as they arise and carries out tasks as directed by providers.
  13. Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow-up with the focus on attaining treatment goals.
  14. Keeps abreast of new information and materials by reviewing clearinghouse information, newsletters, and periodicals.
  15. Researches and designs educational materials as indicated by best practices and patients’ needs to assist in developing and obtaining personal health goals.
  16. Other duties as assigned.

Duties 1 through 15 are designated as ADA Essential Functions and must be performed in this job. All other job duties are secondary functions.

Qualifications: Training and Education 

  • Nursing degree from an accredited college/university is required OR
  • Bachelor’s degree in social services or related field from an accredited college/university is required.

Qualifications: Experience 

  • Two years of nursing experience in a clinical setting or case management is preferred; AND/OR
  •  Experience in community-based behavioral health care and case management is preferred.

Qualifications: Licensure/Certification

  • Must have a valid Nebraska driver’s license and provide proof of personal driver’s insurance and the ability to utilize own transportation for work purposes.
  • A nurse must have a current unrestricted license as a Registered Nurse (RN) or a Licensed Practical Nurse (LPN) in the State of Nebraska.
