CWOCN certification by Wound, Ostomy Continence Nursing Certification Board (WOCNCB) or pursuit of certification within 1 year.
Current Texas RN license
High level of energy and motivation
Must be able to work with minimal supervision following probation period
Work Experience: Two (2) years recent, relevant nursing experience as licensed BS RN

 
Your Job Responsibilities:
 

Assuring the Program objectives are successfully implemented and coordinated for all wound, ostomy and continence issues including but not limited to wound, ostomy and continence skin issue assessment, plan of care development, coordination of care, and discharge planning for patients with wound, ostomy and continence issues. 
Assists Wound Committee in development of Standards of Practice for managing patients with wounds, ostomy and incontinence issues.
Works with educators to educate and train the nursing staff on wound assessment, treatments, reporting and documentation of progress. 
Works directly and in collaboration with the physicians and coordinates schedule with team members appropriately and efficiently as part of the Wound Care Team for Methodist Health Systems.
Responsible for relationships with wound, ostomy and continence care product vendors, central supply services, and other Methodist Health Systems wound care teams and projects as they relate to inpatient and outpatient Wound Care Program.
Maintains approved National Specialty Certification and advanced training in this clinical specialty.  This role is expected to support the vision, mission, and strategic plan and goals for Methodist Health Systems.

I. Primary Function

II. Primary Duties and Measurements

1.Wound, ostomy & continence care program functions: 50%

A. Assists with the coordination of the Wound Care Program to include development, implementation, education, and monitoring to assure efficient, effective, quality wound, ostomy and continence services.
B. Communicates with all appropriate entities regarding the Wound Care Program to include, but not be limited to, Nursing, Physicians, Wound Care Center, and Allied Health Departments such as Central Supply, Clinical Nutrition, Social Work, Physical Therapy, Risk Management, and Medical Supply Vendors.
C. Coordinates with the Materials Department to maintain appropriate wound, ostomy and incontinence care supplies/products and equipment to support wound care operations.
D. Serves as a Wound, Ostomy & Continence Nurse Specialist liaison/leader to promote professional relationships within Nursing, Medical and hospital staff. Provides good customer service throughout all interactions.
E. Provides wound, ostomy and continence care outcome data and analysis to assess effectiveness of Wound Care Program. Modifies and advances Wound Care Program as appropriate based on clinical outcomes.

Participates in Quality improvement projects as needed.

Performs other duties as assigned.

2. Assessment: 45%

A. Performs advanced, proficient, and comprehensive wound, ostomy and continence care assessment.

3. Planning:
A. Develops and coordinates comprehensive wound, ostomy and continence plan of care based on assessment, including primary care nurse involvement, physician consultation, treatment plans, and nursing/patient education. To include but not limited to: Product selection, Advanced/complex modalities, chemical cauterization, wound debridement and evaluation of progress and measurement of outcomes [within scope of practice].
B. Stoma site selection including pre-op and post-op education and management, fitting of appropriate ostomy appliance prior to discharge, treatment of peristomal skin complications; sexual, dietary and vocational counseling.

4. Implementation:
A. Demonstrates clinical proficiency in wound, ostomy and continence care management.

5. Evaluation:
A. Monitors, modifies, and advances the wound, ostomy and continence care plan based on re-assessment, consult recommendations and clinical outcomes.

B. Educates nursing, patient, and family regarding the wound, ostomy and continence plan of care, goals, and discharge recommendations.
C Completes all documentation including occurrence reporting in a timely manner per hospital guidelines.
D. Communicates with healthcare team throughout patient's stay to maximize coordination of wound, ostomy and continence care plan and clinical outcome. Utilizes appropriate verbal and written communication methods to facilitate optimal patient and family learning.

III. Professional Practice
1. Critical Thinking, Decision Making, and Professional Judgment 15%

A. Manages daily patient schedule to include new assessments and existing wound, ostomy and continence issues reassessments. Coordinates schedule with team members appropriately and efficiently.
B. Completes all administrative wound care program and patient care activities in a timely manner and per hospital guidelines.
B. Makes effective decisions utilizing critical thinking skills and professional judgment regarding patient care, collaboration with healthcare team, unit operations and patient/customer satisfaction.
C. Performs other delegated activities as directed within the time frame given.
D. Completes all annual mandatory education and licensure requirements and pursues ongoing educational opportunities for professional development.
E. Participates and supports MHS and unit based/department goals.
