The Community Health Worker (CHW) is a front line public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. The clinic-based community health worker will be based in a Mercy primary or specialty care clinic, working with individual patients as part of an integrated team of health care providers. This position is a temporary position that has the possibility of becoming permanent. Coworkers may also have the opportunity to explore other options within Mercy if position is not made permanent.
- Strong, trusting relationship building and collaboration skills.
- Excellent communication and facilitation skills.
- Patient advocate between patient and community services such as schools, Department Human Services, hospitals, etc.
- Non-clinical coaching/education of patients/families using motivational interviewing.
- Ability to work independently and also function as part of a team.
- Basic knowledge of community resources such as transportation, medication assistance, utility assistance, food pantries, etc.
- Assisting with the completion of applications/registration forms.
- Good time management skills.
- Attention to detail.
- Attendance in staff meetings, trainings, and other meetings as requested.
- Computer and typing skills.
- Ability and willingness to provide emotional support, encouragement, and motivation to patients.
- Compassion for the poor and underserved members of the community.

- Associate’s degree OR 2 years of college required. Bachelor’s degree is preferred.Successful completion of Community Health Worker (CHW) formal training program is preferred.
- Knowledge of some medical terminology is preferred
- Two or more years in any health setting
- Bilingual preferred
