Job Description


The Case Manger assists youth clients and their families with access to social and community support services. Coordinates outreach and referrals of services utilizing extensive knowledge of community resources for family stabilization to include health care, mental health, legal aid, educational support, financial assistance, and other services.   



  • Conduct intake interviews and complete individual assessments of families’ needs, including demographics and level of comprehensive services required.
  • Prepare progress notes, family service plans, and other records and documentation.
  • Develop strong rapport and supportive, nurturing relationship with children and families.
  • Coordinate proposed family service plan with providers and facilitate referrals.  Schedule appointments and arrange transportation.  
  • Provide case management services for the family in the home as needed.
  • Maintain a caseload.  Maintain contact with treatment providers to continuously appraise client progress.
  • Provide crisis intervention.
  • Communicate regularly with client families as required to provide needed support.
  • Facilitate support groups, youth groups and ILP classes as needed.
  • Comply with all MHS and program rules, policies and procedures including HIPAA standards.



  • 3+ years’ experience in a case management setting preferred 
  • BA in social work or related field required 
  • Bilingual, Fluent in English and Spanish, required
  • Proficient with Microsoft Office Suite, regularly work with and understand Excel
  • Ability to work independently and effectively collaborate with a team
  • Able to problem solve and react appropriately to emergency situations.
