Job Description

Memorial Community Health is hiring for full time staff hospital pharmacist for 4 days per week position from 7 am to 6 pm.  No weekend, holidays, or on-call time.  Work at critical access hospital in rural community of 4,500 people

Responsibilities:

  • Reviews and interprets medication orders to assure accuracy and determines formulas and ingredients needed. 
  • Provides accurate, safe, timely, and appropriate medication therapy based on patient age and needs. Preforms medication distributive functions, including, but not limited to, preparation and/or verification of medications, labeling, compounding, and dispensing.  Prepares IV admixtures and parenteral products under aseptic conditions.
  • Provides pharmaceutical services which include order review and verification, pharmacokinetic dosing/monitoring, renal dose adjustment, anticoagulation monitoring, and IV to PO conversion.
  • Completes critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug-related problems, improper drug or dose selection, sub therapeutic dosage, over dosage, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication, and treatment failures.
  • Works as an active member of multidisciplinary team and collaborates with healthcare providers to provide patient-centered care.  Acts as drug information resource for prescribers, hospital staff, and patients.
  • Communicates effectively and appropriately with healthcare providers and caregivers (physicians, nurses, etc.) and assures continuity of pharmaceutical care. 
  • Provides discharge medication review, reconciliation, and counseling as appropriate
  • Maintains established procedures concerning quality assurance monitoring, security of controlled substances, and disposal of hazardous medications.
  • Clinically supervises pharmacy technician activities and performance.  Provides direction and guidance as needed. 
  • Participates in the orientation, instruction and training of pharmacy students and interns, pharmacy technicians, residents, pharmacists and other health professionals.
  • Actively participates in antimicrobial and opioid stewardship activities and medication error prevention programs.  Helps to design and implement stewardship activities and restriction/surveillance programs.
  • Assists in education and teaching programs (departmental, medical, nursing, community) as requested.
  • Maintains proficiency in hospital computer systems and medication ordering systems
  • Facilitates specialty medication procurement, ordering, and dispensing procedures including but not limited to chemotherapy, parenteral nutrition, controlled substances, etc. as appropriate for assignment
  • Endorses policies and procedures of the department and of the facility.

 

Qualifications and Education:

Current Pharmacist license from the State of Nebraska required

Bachelor’s degree in Pharmacy or Doctor of Pharmacy degree

Pharmacy experience (1-2 years) in a hospital setting preferred but not required
