Lead Mobility Assessment Evaluator

The Honolulu Team has an exciting opportunity for a Lead Mobility Assessment Evaluator.

The Lead, Mobility Assessment Evaluator (MAE) is responsible for conducting interviews, physical functional assessments and cognitive assessments with applicants to determine eligibility for ADA paratransit service. This position is responsible for paperwork, documentation, and final eligibility determination associated with applications, re-applications and re-certifications. The Lead, MAE is also responsible for assisting the Site Manager with reviewing cases prior to submission to the Client, assisting with Staff meetings and Staff scheduling, stepping in to assist with location management in the event the Site Manager is out of the office.

This position is located at our office in Honolulu, HI.


  • Serve as the team’s Subject Matter Expert, both locally and externally, and educate employees on new processes and procedures
  • Act as a mentor to new employees and assist with the development of team members
  • Determine what functional assessments the applicant will be required to undergo
  • Make the final determination regarding eligibility based upon the application, interview, functional assessment and any supplemental information
  • Work with the Travel Training team and Contact Center team to ensure program effectiveness
  • Responsible for overseeing the day to day operations for the site
  • Conduct in-person interviews to assess mobility capabilities and review any supplemental information provided
  • Review interview results and application paperwork
  • Conduct functional assessment with applicants that may include:
    • Physical Functional Assessment using an outdoor course
    • Physical Functional Assessment using an indoor course
    • FACTS test
    • MMSE
    • Tinetti Balance test
  • Complete all necessary reports and paperwork
  • Input all information into MTM Access and other software as appropriate
  • Build rapport and communicate effectively with applicant
  • Maintain a positive attitude when interacting with individuals, support staff and caregivers
  • Provide high quality customer service
  • Must conform to MTM’s HIPAA standards
  • Assist with Quality Assurance

Minimum Qualifications:

  • Bachelor’s degree in Social Services, Rehabilitation, Ancillary Health Care Profession, or a related  field
  • Minimum of three years of professional experience working with individuals with  disabilities
  • Minimum of one year experience conducting assessments and making final determinations for eligibility
  • Experience conducting information-gathering interviews or assessments
  • Familiarity with ADA regulations and sensitivity to issues relating to people with disabilities and older adults
  • Intermediate level of proficiency or above with the Microsoft Office Suite (Outlook, Word, PowerPoint & Excel)
  • Familiarity with the local public transportation system
  • Ability to work flexible hours and adapt to a rapidly changing environment
  • Additional language skills are preferred

Competency Based Qualifications:

  • Knowledge and ability to perform and oversee evaluations
  • Experience conducting information-gathering interviews or assessments
  • Solid interviewing, assessment and observational skills and demonstrated ability to work  effectively with older adults and people with a wide range of disabilities
  • Ability to interact effectively with individuals with of varying levels of intellectual and cognitive functioning disabilities
  • Ability to be trained in administering a standardized evaluation tool and be able to consistently administer and score such a tool according to established protocols
  • Ability to clearly communicate their findings and observations in written and verbal form on the required database reports and notes, as well as in certification letters
  • Ability to learn additional programs
  • Excellent organizational and multi-tasking skills
  • Ability to maintain high level of confidentiality
