Job Description

JOB SUMMARY:

The LPN provides basic nursing care. And work under the direction of the registered nurse and Medical Provider. The LPN is responsible for the care and services provided patients at Medical Associates Plus. The position is also responsible for collaborating with clinical, clerical, administrative, and support staff to facilitate the care and progress of the patient through the recovery process.

DUTIES & RESPONSIBILITIES:

  1. The position requires initiative, team skills, communication and interaction with health care management and the other health care providers.
  2. The LPN provides holistic patient care in high intensity outpatient setting by applying technical nursing skills, and professional judgment to a widely varied patient population including all ages: infant, child, adolescent, adult, and geriatric. Administer medications according to department policy and performs venipuncture for lab orders as ordered by the medical provider.
  3. Always work as a team player to provide quality patient care, whether direct or indirect. Within scope of licensure, each employee will assist other team members in accomplishing their job duties to get the job done.
  4. Holds self and team members accountable for knowledge of and full compliance with customer service performance standards as listed on all team members’ job descriptions.
  5. Always participates in the efficient and effective management of the company resources.
  6. Supports and participates in quality improvement activities.
  7. Performs triage on patient for the collection of vital signs (Pulse, Temperature, Blood Pressure, Weight, Height)
  8. Gather patient information and keep records updated with the relevant data.
  9. Maintain effective communication with the medical personnel, and patient.
  10. Administration of medication by oral, injections and topical route.
  11. Provides and interpretation of clinical policies and procedures regarding nursing and medical assistant policies and procedures.
  12. Assist in the designs and implements tools for assuring and measuring patient processes and nursing services.
  13. Advises or assists in patient complaints specific to nursing/medical assistants/clinical issues.
  14. Participates in the Continuous Quality Improvement activities with follow-up on areas requiring corrective action.
  15. Maintains and recommends updates to clinical indicators and clinical procedure manuals.
  16. Consults with the DON/CMO concerning clinical issues and problems.
  17. Consults with the DON concerning staffing schedules to provide efficient and cost-effective patient care coverage.
  18. Assists the DON in development, implementing and monitoring, an effective new patient care employee orientation and training process.
  19. Provides input into the recruitment and hiring process for patient care personnel.
  20. Assists the DON/CMO to ensure patient care standards are met and in compliance with grant process, quality improvement, legal aspects, and other regulatory requirements.
  21. Attends required corporate meetings and participated in committees as requested by the CEO and CMO/DON.
  22. Assists and provides in-service education on selected topics as directed by CEO or CMO/DON
  23. Attends clinic staff meetings and responds to written and/or verbal questions and concerns in a timely manner.
  24. Responsible for completion of special assignments as assigned by CEO or CMO/DON.
  25. If assigned, reviews clinical supply requisitions and help with inventory control.
  26. Supports and promotes the goals of the Neighborhood Improvement Project, Inc.
  27. Participates in strategic planning for NIP.
  28. Verifies at least annually nursing and medical assistant staff competencies.
  29. Assists with reports as requested.
  30. Assists with accreditation processes as requested.
  31. Monitors/audits nursing responsibilities including medical records documentation, referral logs, emergency equipment, quality control of instruments, triage process, standing orders, and patient education.
  32. Participates in nurse/medical assistant team meetings to address current issues.
  33. Works with DON to provide support and positive outcomes as well as conflict resolution. Makes recommendations to patient care problems and complaints through incident reporting.
  34. Participates in professional development activities as needed to keep updated on medical/clinical expertise.
  35. Provides Case Management services as directed.
  36. Assures appropriate management of vaccines, training on GRITS and submission of required reports. Maintains provider profile agreement.
  37. Coordinates the provision of educational in-services to clinical staff on new immunization, VFC and GRITS updates.
  38. Other duties as assigned.


EDUCATION:

Graduate from an accredited LPN program. LPN programs generally include one year of coursework and practical application at a hospital, vocational technical school, or community college.
