The Registered Nurse (RN) Care Coordinator works within an interdisciplinary team to coordinate care for a defined population of patients. The goals of care include supporting improvement in health outcomes, empowerment of patients and families to engage in health seeking behaviors and disease prevention, and supporting self-management of chronic conditions.
Care is provided in a holistic, patient-centered approach across settings spanning the continuum of care, with an emphasis on assisting patients to seek an appropriate level of service when needed and reducing the overall cost of care.
The RN Care Coordinator will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for an assigned population of patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN. The American Academy of Ambulatory Care Nursing Scope and Standards of Practice for Registered Nurses in Care Coordination and Transition Management (2016) are reflected.
