Patient Care Coordinator
Position Summary:
The Patient Care Coordinator (PCC) oversees, participates, and carries out responsibilities which ensure high quality, well-coordinated patient care for Little River Medical Center’s self-monitoring patient care programs (SMPCP). Objectives of the self-monitoring patient care programs are to support patients with managing their chronic condition(s) by providing patients with an at home device to monitor targeted vitals related to the chronic condition(s) and to provide ongoing communications with the patients and providers of outcomes while participating in the program.
Essential Duties and Responisbilites: Includes the following, but not limited to:
  • Coordinates and implements self-monitoring patient care programs in conjunction with leadership.
  • Participates in program meetings, staff meetings and trainings and assists in coordination or provision of continuing education for clinical or support staff.
  • Works in the capacity as a direct patient care manager for one or multiple programs depending up on the demands of the program(s) includes, but not limited to hypertension and diabetic management (TACM).
    1. Ensures the collection of statistical and program data for dashboards, UDS reporting requirements, and other tools utilized for measuring program performance and patient outcomes.
    2. Manages the supplies inventory for the program and the allocation of the methods designated for the self-monitoring patient care program(s).
    3. Recruitment, enrollment, and education of patients into the SMPCP that includes device setup, device support, monitoring and communicating readings between the patient, the provider, and clinical team.
    4. Maintains accurate records of readings and documentation in the electronic medical record.
    5. Collaborates with Administrative and Quality Improvement team in clinical program goal setting, progress evaluations and ongoing problem solving.
    6. Participates and assist in organizing patient related program meetings, participation in community events, health fairs, and other external opportunities to promote SMPCPs.
  • Participates in ongoing electronic medical record (EMR) training and assists the Health Information Systems Coordinator in optimized development and use of the EMR system with respect to self-monitoring patient care programs.
  • Maintains excellent customer service relationships with patients, colleagues and outside partners.
  • Represents Little River Medical Center, professionally in all work related interactions with vendors, community members and health care colleagues.
  • Performs all other duties as assigned.
Education and/or Experience:
Licensed Registered Nurse or Licensed Practical Nurse with a minimum of 2 years in a clinical setting, required. Experience with quality programs and measuring patient outcomes is preferred.
LANGUAGE SKILLS
Knowledge and skills of the Spanish language is a plus but not required.
CERTIFICATES, LICENSES, REGISTRATION
Licensed Registered Nurse in the state of South Carolina or Licensed Practical Nurse in the state of South Carolina.
