Job Description

Lightyear Health is looking for a fully-remote Patient Care Coordinator to support Physical Medicine & Rehabilitation services for equitable access to high-quality care.

To help us achieve our mission, we are looking to hire a Patient Care Coordinator who follows through on patient care and envisions a world where every aging patient has access to the world's best healthcare. This is a full-time remote role serving patients and will report to our Practice Manager.

As a key member of our Clinical Operations - Physical Medicine and Rehabilitation team, here's what you'll do:

Key Outcomes:

  • Be a liaison daily between our patients, patient stakeholders, colleagues, and external parties
  • Provides information on the scope of Lightyear Health's services including the physician medicine & rehabilitation (PM&R) service offering, in-person skilled nursing facility (SNF) rounding model and value-add of services
  • Supports PM&R clinician team daily with pre-access activities for PM&R Skilled Nursing Facilities
  • Virtually rooms and interviews patients by gathering medical, mental, and social history for individualized care plan
  • Reliably documents PHI into patient's EHR, and maintains communication with clinician team to ensure chart accuracy and completeness, in accordance to HIPAA compliance
  • Validates patient's medical insurance benefits, confirms prior-authorization or referral needs by collaborating with Revenue Cycle team

In addition to the above, it would be great if you also had: 

  • At least 2 years of experience in one of the following role types: Medical Assistant, Medical Administrative Assistant, Patient Care Coordinator, Unit Assistant, Patient Services Representative, Patient Access Representative
  • At least 2 years of experience working in a medical setting (e.g. you've supported clinicians, interacted with patients and have used an EMR before)

Compensation: 

We're committed to an inclusive, consistent, and equitable approach to compensation and anticipate that this position will earn between $42,000-$50,000 annually. The exact salary will depend on (1) how many years of relevant and transferable experience you bring to the role and (2) the internal level at which you are  hired in relation to our career development framework. All full time team members also receive meaningful equity in the form of a stock option grant.
