residents, families, visitors, and vendors.Position Functions and ResponsibilitiesClient Care & Service Delivery Provides direct client care as defined in the state Nurse Practice Act. Implements plan of care initiated by the registered nurse. Provides accurate and timely documentation consistent with the plan of care.
Monitors and provides client and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care. Assists with monitoring visits as directed by the RN Case Manager to evaluate ongoing client needs and quality of care Completes nursing focused assessments during personal cares; document results; escalate to RN Case Manager as necessary for follow-up Attends MD appointments as requested Assists with skilled services as directed by care plan Coaches, mentors, and supports unlicensed staff as needed. Timely completion of required forms, reports, orders.
Assist clients with implementation Activities of Daily Living and Delegated Tasks according to clients' needs, wishes, and preferences outlined on the Care Assignment Review and implement Care Assignment Performs Personal Cares (bathing, dressing, grooming, toileting, skin care, etc) Lifting, Transferring (pivot transfer, 2-person, Hoyer lift) Range of Motion & Positioning Exercise & Ambulation Dining & Nutrition Homemaking services Vital Signs Medication Reminders and Administration Special Tasks Fall Prevention Observing and Reporting status updates and changes Following all documentation requirement Follows infection control and OSHA policies and procedures Other duties as assigned by the RN Case ManagerVisits, Communication, and Customer Services Lead by example in establishing rapport with clients, clinicians, partners, and the community. Ensure positive/professional communication within the team Customer service resolution; escalate concerns Prepares clinical notes and updates the primary physician when necessary and at least every 60 days. Communicates with the physician regarding the client's needs and reports any changes in the client's condition; obtains/receives physician's orders as required.
Communicates with community health related persons to coordinate the care plan.QualificationsExperience Prefer minimum of 1 year of nursing experience Minimum of 1 year of caregiving experience (assisted living, home care, hospice) Minimum of 1 year experience working with people in the second half of life.
