Liberty Resources, Inc., is seeking a licensed therapist to provide Community Psychiatric Support and Treatment (CPST) under the HCBS model to individuals living in Onondaga County. CPST includes time-limited goal-directed and solution-focused interventions intended to achieve the goals and objectives outlined in an individual’s Treatment Plan. CPST is designed to be provided in the community to individuals who are unable to receive site-based care and is intended for individuals being discharged from inpatient units, individuals with a history of non-engagement in services, individuals who are transitioning from crisis services, and individuals who have disengaged from care. The therapist is responsible for outreaching, engaging, and providing in home therapy and supportive services to individuals as outlined in their Plan of Care and individual Treatment Plan.
Assist individuals with identifying strategies or treatment options to address the mental health and/or substance use needs using evidence-based and best practice techniques.
Provide supportive counseling to individuals and their family using solution focused interventions, emotional and behavioral management, and problem behavior analysis with the individual to restore stability, to support functional gains, and to adapt to community living.
Facilitate participation in and utilization of strengths based planning and treatments which includes assisting individuals and family members with identifying strengths and needs, resources, natural supports, and developing goals and objectives to utilize personal strengths resources and natural supports to address functional deficits.
Assist individuals with effectively responding to or avoiding identified precursors or triggers that would risk their remaining in a natural community location including the development of a crisis management plan.
Provide ongoing rehabilitation support for individuals pursuing employment, housing, or educational goals.
Build relationships and collaborates with clinical treatment providers, care coordinators, and other supports to determine continuation of care and makes recommendations for modifications to an individual’s Plan of Care.
Establish working relationships with various Managed Care Organizations (MCOs), ensures compliance with Medicaid’s HCBS billing requirements, and obtains authorization for services as needed.
Practitioner possessing a license or a permit from the New York State Education Department who are qualified by credentials, training, and experience to provide direct services related to the treatment of mental illness and substance use disorders including the following: LMSW, LCSW, LMFT, LMHC, RN, NP, CRC, LCAT, and Licensed Psychologist.
Sensitive to cultural differences and able to incorporate that understanding into service provision.
Demonstrates initiative, sound judgment and understanding of clinical issues affecting individuals and families in the mental health and substance use system.
Must be able to work independently and with flexibility in days and hours of work.
