Job DescriptionLegacy House of Avondale is seeking a compassionate and skillful LPN to join ourteam, as the Wellness Director.Caring for seniors and their families since 1996, Legacy Retirement Communities is a premier provider of senior living services. Located within Pima Center at the Loop 101 between Via de Ventura and Pima Street, Legacy House of Avondale is Arizona's premier senior living community.PRIMARY JOB DUTIES Design and deliver superior nursing care to community residents. Performs a through health assessment on all new and existing residents.

Monitors the health status of all resident's and participates in care needs. Completes and maintains current resident service plans. Service plans must be unique to the resident and based on the most recent assessment.

Develops and maintains community drug management programs. Trains and monitors nurses and unlicensed staff in the administration and management of medications. Develops staff training programs and...

trains staff according too corporate and state guidelines. Arranges for training from outside vendors. Ensures compliance of all community staff with corporate and state training requirements.

Document periodically residents' condition and noting significant changes in condition when they occur. Respond appropriately to residents or family requests. Participate in resident service planning meeting, quality assurance meeting, or other appropriate meetings as requested.

Develops, implements, and monitors the community's Employee Health Program. Ensure compliance with all rules, regulations, laws and statutes pertaining to the licensing of an assisted living community.JOB QUALIFICATIONS Graduated from accredited nursing program. Minimum 1-year management in assisted living or long term care.Current LPN license with the state of Arizona.

CPR/ First Aid Certifications. Knowledge of quality nursing practice. Ability to direct and lead staff in multiple tasks.
