RN Case Manger, Discharge Planner

The Discharge Planner participates in a multi-disciplinary planning program operated under policies outlined in the Discharge Planning Policy Manual. The Planner must be able to positively interact with all disciplines involved and also with other hospital departments to assure most appropriate and efficient discharge possible.

General Responsibilities: Observe professional ethics in maintaining confidential information acquired concerning the personal, financial, medical or employment of patients of Lawrence Memorial Hospital. Must follow and enforce accepted Patient Safety practices. Must report safety hazards and initiate appropriate action. must participate in safety instructional programs. Good communications skills with ability to notify physician and/or ambulance if necessary. Excellent verbal and written skills to facilitate communication with a variety of medical and technical personnel. Must demonstrate knowledge of the principles of growth and development over the life span to his/her age specific needs and to provide the care needed.


Graduate of accredited school of nursing and licensed by the state of Arkansas.
A College degree in Nursing with experience in Case Management/Discharge Planning and hold a current Arkansas license. Must have knowledge of medical records, documentation, nursing assessments and medical diagnosis. Must also understand the financial reimbursement under the Prospective Payment System, Utilization review procedures, ICD-ICD-9 coding and medical terminology. Participates in continuing education, i.e., seminars, workshops and educational materials. Computer skills for data collection, comparative analysis, graphic software, word processing and spread sheet usage.


At least one year clinical experience.

License/Certification(s): Current Arkansas RN license, BLS certification. NIH, ACLS, PALS and TNCC certified within 6 months of hire.
