Licensed Practical Nurse Reports to: Wellness Director, Care Coordinator Purpose of Position:The primary purpose of your position is to provide routine nursing and nursing related services in accordance with established policies and procedures, nursing practice guidelines, Federal and States laws and requlatory requirements, under the direction of the Nursing Supervisor, Director of Nursing or his/her designee. It is expected that all care provided would be the highest degree of quality resident care that can be maintained. Treat residents, coworkers and visitors with dignity and respect.Report all incidents and accidents to the nursing supervisor, physician and responsible party immediately regardless of how minorReport all hazardous conditions immediatelyPersonally responsible to attend and participate in all available in-service and educational programs and other periodic scheduled meetings. If unable to attend, must make arrangements with the Staff Development Coordinator to obtain information from such meetings and inservices. Major job duties:Report on and off duty to your unit, on time for your scheduled shiftParticipate in change of shift narcotic count and transfer of keys. Report any discrepancies or concerns to the Director of Nursing immediatelyReceive assignment and verbal reportSupervise, guide and direct the work performance of all nursing assistants, unit aides and light duty staff assigned to your team each day/shiftMake assignment and changes to the assignment for nursing assistants, unit aides and light duty staff, clearly communicating the same to those individualsDocument changes to staffing according to facility procedure and replace call-offs in a timely fashionReceive ongoing and end of shift report from assigned nursing assistantsProvide each person under your direction a clear verbal report of responsibilities, resident condition and changes, rehab schedule, whirlpool schedule and appointments daily at the start of the shiftParticipate in change of shift rounds to ensure a smooth transition and continuity of carePerform all job duties in accordance with established policies and procedures, safety rules and regulatory requirementsImplement the nursing process for optimal quality of care and evaluate effectivenessVerification of resident and introduction of self prior to any administered care, medication, treatment or serviceMaintain responsibility and accountability for all residents and sponsors assigned to your care each day/shiftAssist in the instruction and oversight of nursing assistants, unit aides and light duty staff in the delivery of new and/or changes in resident care including rehabilitative, restorative, supportive services, medical orders and plan of care changesAssist in the coordination of all nursing functions of the assigned team assuring the maintenance of physician’s orders, plan of care, rehab, restorative and supportive servicesEvaluate the work performance of nursing assistants, unit aides and light duty staffCommunicate with nursing assistants, unit aides and light duty staff any concerns with work performance, missed duties or other concerns in a timely fashion. Provide guidance for improvement in those areas and communicate promptly any unresolved issues to the Director of NursingInitiate and implement disciplinary action as necessary and appropriate with nursing assistants, unit aides and light duty staff, seeking guidance from nursing supervisors when neededAssume the responsibility and authority of the nursing supervisor at the guidance and discretion of the Director of Nursing and/or his/her designeeReceive, investigate and resolve, as able, employee, resident and family suggestions, concerns or complaints in accordance with facility open door policy and grievance proceduresParticipate in the daily assessment of the long-term needs of assigned residents in regard to total care managementAssist in conducting routine and frequent assessments of assigned residents reporting any changes, new observations, response to treatment, etc. to the nursing supervisor, physician and family according to resident need and facility policy and procedureMonitor and respond to resident nutritional needs, weight gain and loss, assuring that the necessary assistance is provided to meet the nutritional and hydration needs of each resident, including the use of adaptive equipment as ordered and appropriateMaintain a clear working knowledge of rules and regulations for medication and treatment administrationAdminister physician ordered medication according to licensure and safe nursing practice, documenting all administration at the time of administration according to facility policyAdministers “prn” medications as needed and according to physician order documenting administration and reason for sameLicensed nurses who have a current IV certification may initiate IV access and administer IV fluids according to the state of Ohio IV certification guidelinesMaintains knowledge of the action and adverse action of medications administered. Recognizes contraindications of medications administered and reports concerns immediately. Accesses resources to maintain knowledge in these areasAdminister physician ordered tube feeding to assigned residents according to licensure and safe nursing practice, documenting administration according to facility policyAdminister physician ordered treatments, including dressing changes, oxygen and respiratory therapy to assigned residents according to licensure and safe nursing practice, documenting administration according to facility policyConsults with treatment nurse status and effectiveness of ordered treatments. Reports all changes in skin integrity and completes a skin grid for newly identified woundsComplete organized, thorough and professional nursing documentation in a timely and problem-oriented fashion and according to documentation schedule and facility policyConduct a thorough nursing assessment upon admission, return from leave of absence or change of condition and as appropriateParticipates in the initial and periodic assessment of pain, reporting findings to the Director of Nursing, Assistant Director of Nursing and/or Physician for timely and appropriate treatment interventions to optimize comfort and quality of life while minimizing or eliminating painDocument pertinent clinical information, admissions, transfers, discharges and deaths on the 24 hour report each shiftStimulate interest and encourage resident participation in activities, religious programs, therapies and beauty shop allowing for personal preferences and freedom of choiceAttend, participate and coordinate nursing assistant attendance in plan of care meetingsMaintain ongoing communication with other disciplines to ensure resident needs are addressed and metCommunicate any changes in care, medication, treatment or condition to the resident and responsible party and according to facility policyExecute all appropriate nursing
