We are seeking a Consulting Community Based Registered Nurse to join our team! You will be responsible for the assessment, diagnosis, and treatment of Behavioral Health assigned patients. Must have own transportation, cell phone and computer for documentation & billing. $50.00 per billable hour. Must have a DC License.

Responsibilities:

· Diagnose and establish patient treatment plans

· Monitor and report changes in patient symptoms or behavior

· Communicate with collaborating physicians or specialists regarding patient care

· Educate patients about health maintenance and disease prevention

· Facilitate referrals to other healthcare professionals and medical facilities

· Maintain accurate patient medical records

· Provide advice and emotional support to patients and their family members

As a consultant for the provision of behavioral nursing care services, the incumbent provides patient care by administering treatment and medications, performing diagnostic assessments and arranging patient follow-up; and record consumers' medical histories and symptoms. The Registered Nurse is assigned to meet with consumers in their residence and conducts a continuing and comprehensive practice that is preventive, curative and rehabilitative. The philosophy of care is based on the belief that care directed to the individual recovering from psychiatric disabilities contributes to positive health care outcomes. Participation with assigned Core Service Agency consumers is encouraged for the development of community activities that contribute to the promotion of education about and maintenance of good health. The Consulting Community Based Public Health Registered Nurse is one of the members of a team of individuals whom are assigned to the consumer. Most services are provided in the evenings and on weekends. $50.00 per billable hour

The goal for this service is to Increase the quality, availability, and effectiveness of educational and community-based programs designed to prevent disease and injury, improve health, and enhance quality of life. Educational and community- based health educational activities and services play a key role in: preventing disease and injury; improving health; enhancing quality of life. Health status and related health behaviors are determined by influences at multiple levels: personal, organizational/institutional, environmental, and policy. Because significant and dynamic interrelationships exist among these different levels of health determinants, educational and community-based programs are most likely to succeed in improving health and wellness when they address influences at all levels and in a variety of environments/settings.

The overview of the responsibilities is inclusive of, but not limited to:

· Track and assure completion of required patient assessments and screenings, including (Patient Health Questionnaire (PHQ-9), Abnormal Involuntary Movement Scale (AIMS) and other assessments deemed appropriate to meet patient needs;

· Review, analyzing and ensuring the availability of laboratory reports;

· Executing medical regimens as identified by written physician orders and Medical Administration Reports (MARs);

· Patient health counseling and instruction

· Completes medication reviews and conducts a medication reconciliation with input from the consumers primary care physician;;

· Participates in the development and implementation of an integrated-person-centered care planning process related to physical health care needs of assigned consumer;

· Conducts assessments of the consumer’s health needs;

· Provides educational training on chronic physical disease states and treatment protocols and, Identifies and assists with the implementation of self-management protocols for use by consumers.

· Monitor and report changes in patient symptoms or behavior

· Communicate with collaborating physicians or specialists regarding patient care

· Educate patients about health maintenance and disease prevention

· Facilitate referrals to other healthcare professionals and medical facilities

· Maintain accurate patient medical records

· Provide advice and emotional support to patients and their family members

· Implements all prescription orders of the Medical Director.

· Ensures that all orders are updated every 30 days.

· Actively participate in the treatment team meetings.

· Develop and implement educational classes on self-administration of medication.

· As members of the treatment team, the Nurses will assist in crisis intervention. and will be available for on-call rotation.

· Advocates for a clinical philosophy which promotes client access to ethical, competent, effective care.

· Monitors all medication administration ensuring that the policies and procedures are strictly adhered to.

· Ensures the integrity and consistency of the medication administration and medical education.

· ENSURES the policies and procedures for nursing duties and medication administration meet all contract, state and federal guidelines and are properly implemented in each of the Life Stride sites.

As a consultant for the provision of nursing care services, the incumbent provides patient care by administering treatment and medications, performing diagnostic assessments and arranging patient follow-up; and record consumers' medical histories and symptoms.

Educational and community-based programs encourage and enhance health and wellness by educating communities on topics such as:

· Chronic diseases; Injury and violence prevention; Mental illness/behavioral health; Unintended pregnancy; Oral health; Tobacco use; Substance abuse; Nutrition; Physical Activity; Obesity prevention ·

Emphasis for service delivery by the RN is important when for those consumers with mental illness along with one or more chronic physical conditions needs assistance for the management of chronic physical conditions such as heart disease and diabetes which requires significant modifications to the lifestyle and diet of the patient.

The RN will provide services to consumers in their homes and in groups for: supervision or administration of medications; monitoring and interpreting results of laboratory diagnostic procedures related to medications; medical interventions needed for effective mental health treatment; monitoring the side effects and interactions of medication and the adverse reactions which a patient may experience; providing education and direction for symptom and medication self-management; psycho-educational programming as appropriate for consumers and their families as appropriate, regarding the patient’s mental illness, emotional disturbance or behavior disorder, treatment goals, potential benefits and risk of treatment, self-monitoring aids, and education enhancement of the therapeutic alliance between the patient and other service providers. This will include obtaining and working with dieticians to develop appropriate eating habits and menus, while assisting with diets that are based on the financial abilities of consumers to purchase and maintain as newly acquired lifestyles.
