Position: Housing Case Manager
Reports To: Program Manager
SUMMARY: The Housing Case Manager (HCM) assists homeless families to secure permanent housing and provides appropriate supportive services to help them retain housing. The HCM plays an integral role in identifying eligible participants. This position is part of a team and provides innovative services that are at the forefront of homeless services.
ESSENTIAL RESPONSIBILITIES: 1) Complete intake and assessment on all participants identified to determine most appropriate level of housing intervention. 2) Identify and assist participants in securing housing, basic needs, medical and mental health services, substance abuse treatment, and public benefits where eligible; Develop a Housing Stability or Individual Service Plan in conjunction with the participant. 3) Provide strengths-based case management and service coordination designed to assist participants in obtaining and maintaining stable housing. 4) Meet with participants on a weekly basis (more often when needed), to monitor progress and /or barriers toward achieving their Housing Stability or Individual Service Plans. 5) Utilize evidence-based practices in service delivery such as Trauma-Informed Care, Intensive Case Management, Motivational Interviewing, Harm Reduction, and Housing First practices. 6) Maintain accurate and timely records of activities, case management notes, and services provided to each client using HMIS and any other applicable data management system. 7) Develop and maintain relationships with private, public, and nonprofit landlords, including municipal housing departments. 8) Perform site visits of prospective units to ensure they meet guidelines for habitability standards, including lead based paint inspections. 9) Ensure subsidized units are with the Fair Market Rate (FMR) guidelines; Negotiate rent when necessary. 10) Provide advocacy as needed to help address issues and barriers between landlord and participant whenever applicable. 11) Develop and maintain relationships with participants, staff and local service providers. 12) Attend staff and other community meetings as necessary; and serve as a representative of UBH at community events. 13) Prepare and present assigned cases at weekly/monthly Participant Progress Review Supervision Meetings. 14) Provide regular, written reports to the Program Manager and Management as assigned. 15) Other duties as assigned. QUALIFICATIONS: 1) Bachelor’s degree from an accredited college or university in social work, human services or a related field; a combination of education and experience may be substituted. 2) At least 2 years experience providing services to persons experiencing homelessness preferably homeless families. 3) Demonstrated experience in working with homeless families to reach their housing, employment, or education goals. 4) Demonstrated experience in one or more of the following areas: mental health, chemical dependency, emotional/behavioral special education etc. 5) Demonstrated knowledge of and ability to work effectively with persons with a variety of identities, cultures, and backgrounds including ethnic/racial minorities and LGBTQ. 6) Knowledge of Trauma-Informed Care and Practices a plus. 7) Ability to work collaboratively, but independently. 8) Strong written and verbal communication skills. 9) Creative problem-solving skills. 10) Ability to work in a team. 11) Exceptional organizational skills. 12) Bilingual (English/Spanish) preferred. 13) Ability to work with diverse population. 14) Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook). 15) Experience using a data management system to record client information and interactions. 16) Employment with UBH is contingent on completion of satisfactory background check. 17) TB Test Clearance upon hire.
