Salary: $21.00 to $25.00 /hour

Job Type: FULL-TIME 11:00PM - 7:30AM Every other Weekend and Rotating Holidays.

    Job Summary: The LPN is responsible for managing the resident-directed health plan, supervising the CNA's and the day-to-day resident clinical needs during assigned shift. The LPN reports to the Supervisor/Director of Nursing.

    Requirements:

    • Maintain current Delaware nursing license.
    • Valid CPR, First Aid and AED certification.
    • Strong leadership skills
    • Reliable transportation
    • Basic office computer knowledge

    Job Description:

    • Communicate Resident Service Agreement needs with caregiver staff
    • Delegate and supervise direct care tasks with nursing assistants
    • Collaborate with nursing assistants and other members of the care team to ensure highest level of independence for residents
    • Actively participate in nursing meetings, staff in-services and any suggested training as required
    • Assist facility administration in supporting policy and procedure among fellow staff members, residents and guests
    • Build and maintain good working relationships between all departments to ensure quality of care for residents
    • Documentation within the resident clinical records in accordance with facility procedures
    • Create progress note entries to act as part of resident's medical record to document care and treatment provided
    • Complete medication and treatment passes in accordance with state regulation and facility policies
    • Communicate resident / family needs with Resident Service Director, Medical Director and/or Administrator through written reports
    • Meet with residents / families as needed to address questions or concerns
    • Consult with physician for changes in resident condition and regarding matters of treatment, therapy and medications
    • Performing environmental rounds on assigned shift
    • Foster an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the building
    • Complete charting, reports, medical forms and other documentation as required
    • Complete incident reports as needed and perform initial investigation as needed
    • Other duties as assigned

History The Kutz Home traces its roots back to 1902, when the Ladies Bichor Cholem Society was established to visit Wilmington’s sick and elderly. Recognizing the need for a facility to serve as a base for their caregiving, the Society changed their charter to enable the acquisition of property. In 1920, they merged with a similar organization, The Ladies Moshev Zekenim, and purchased a former hospital at 211 West Street in Wilmington. In 1942, they moved to larger quarters on Washington Street. During the 1950’s, the Jewish Federation of Delaware acquired the Sellers Estate in Bellefonte and began planning for a nursing facility. On September 14, 1960, the new Milton and Hattie Kutz Home, a 40-bed facility, opened at its current location on River Road. The needs of the community continued to grow, leading to an expansion in 1970 when 42 beds were added. In 1997, a major renovation and expansion project was completed increasing The Home’s capacity to its present 90 beds, while also adding two new wings, an activities mall, and a visitors center. In 2003 the kitchen, laundry, and loading dock were renovated and expanded, including the installation of a freight elevator. In 2013, Lodge Lane, our beautiful assisted living facility, opened on The Kutz Home campus. With its addition, we are able to provide a much-needed continuum of care. The 1902 vision of the Ladies Bichor Cholem Society still resonates today as we continue to care for our aging and elderly with the reverence they deserve.
