Job Description


The Medical Director is responsible for the delivery of participant care, clinical outcomes, and the implementation as well as oversight, of the quality assessment and improvement program. The Medical Director serves as the lead in the design of the medical components for Senior TLC and as the liaison to the medical community on behalf of Senior TLC. As a physician member of the Senior TLC team, the Medical Director must meet all federal and state licensing laws, certification, and regulations necessary. The Medical Director serves as the consultant and resource to the Primary Care Providers (PCPs). Responsible for NP oversight and supervision based upon the NP and MD collaborative agreement. Collaborates with the STLC SNF Physician Supervisor for care of all STLC participants in SNF, including long-term, short-term rehab, medical and social respite.

Medical Director Qualifications:
Education: Medical Doctor or Doctor of Osteopathy degree required.Licensure: Current or eligible to obtain the State of North Carolina MD or DO licensure required. Current DEA certificate required. Board Certification in Specialty preferred.Experience: Residency in Internal Medicine or Family Practice required, additional training in Geriatrics preferred. At least 2-3 years clinical experience with supervisory and administrative experience with at least 1 years’ experience working with the frail or elderly population.
Minimum: Previous medical office or FQHC experience preferred. Knowledgeable in population health management in the underserved and/or vulnerable populations. Able to communicate and relate well with patients, providers and clinical staff. Efficient, organized and accurate.

Medical Director Key Responsibilities

1. The primary contacts for the Medical Director are the Executive Director, PCPs and other members of the Senior TLC Leadership Team. Serves as a member of the Senior TLC Leadership Team.

2. External Contacts include extensive interaction with representatives of the medical Community. In addition, the Medical Director may interface with the community, state and federal social service, regulatory and other agencies, especially during on-site surveys.

3. Administers the Medial Director’s role in a manner consistent with Senior TLC’s mission, vision, and values. Serves as a member of the Leadership Team and remains cognizant to present as a unified team to the organization.

4. Ensures compliance with PACE provider regulations as they pertain to medical/clinical component of care. Provides monitoring of the Part D program and oversight of the Part D Drug Management Program.

5. Evaluates and implements clinical practice guidelines based on current literature and shared state-of-the-art clinical information with the Senior TLC Interdisciplinary Team.

6. Coordinates the ongoing performance appraisal of the PCPs. Collaborates with the STLC SNF Physician Supervisor for performance appraisal of SNF Advanced Practioner.

7. Supervises assigned Physicians, Nurse Practitioners and other clinical staff in the assessment, initiation, and coordination of direct participant care. Provides internal consultation to the PCPs through case review and analysis of aggregate data.

8. Develops, maintains, and updates internal career path for clinical staff at Senior TLC which incorporates not only clinical education but also performance standards, quality outcomes.

9. Provides direct participant care as required.

10. Liaisons with physician groups, specialist, and hospitals to represent Senior TLC, thereby strengthening positive provider relationships and enhancing referrals. Serves as a liaison between Senior TLC and state and federal agencies as requested.

11. Interfaces with contracted medical providers and facilities when quality concerns arise.

12. Serves on Committees that support the mission Provides input into cost-effective clinical practices.

13. Coordinates all Quality Assurance activities including the development, implementation, and on-going evaluation of a continuous quality assurance plan for the program including but not limited to Performance Goals or Benchmarks: Advance Directives, Service Utilization, Clinical Data Outcome Measures, Pharmacy Utilization.

14. Consults on the development of policies and procedures. Participates in other committees and work groups as assigned.

15. Provides information and conducts presentations as required.
