Job Description

Job Function: To assist cardiologists, PA’s, NP’s and KHH nursing staff with patient care, primarily, in ICU and Telemetry during the patients’ on-going hospitalization and dismissal. Time-permitting, RN may assist in Intermediate or with the patient admission process.

Job Qualifications:

  • Education:
    • Currently licensed to practice professional nursing in the State of Kansas and must be familiar with the basic nursing performance standards as outlined in the Kansas Nurse Practice Act.
    • Graduate of an accredited school of nursing. BSN preferred.
  • A minimum of one year experience in an acute care facility.

Job Duties and Responsibilities

  • Round with Cardiologists and PA’s/NP’s daily in ICU and Telemetry areas
  • Scribe narrative notes and scribe progress notes on patients in ICU and Telemetry
  • Enter CPOE
  • Help facilitate transfer orders and CPOE from ICU to Telemetry floor
  • Determine “possible” dismissals and visit with RN in-charge of patient to initiate the process. Also check with patient’s PA, NP and / or cardiologist
  • Work with KHH Discharge Planner/Care Coordinator in order to coordinate work, duties, avoid redundancy and to get an additional “heads-up” on who may or may not be going home,
  • Time-permitting, roust in ICU and Telemetry
  • DISCHARGE PROCESS / DUTIES:
    • CLINICAL Duties:
      • Ask patient and family to write-down any questions they have concerning their care, medications, procedure(s) or surgery,
      • Ask patient who is their Primary Care Physician and inform them that their information will be sent to their doctor,
      • Make follow-up appointment(s) with a) primary care physician, b) cardiologist, c) surgeon and d) any required consultants. Give appointment date and time to patient and family,
      • Remind the patient and family that they must bring their actual medication bottles to their follow-up doctors’ appointments even if they have a list of their medications,
      • After completing e-discharge summary, go back and review with patient and family any questions they may have,
      • Provide patient and family with business card which will include the RN’s hospital-provided cell-phone number in order to answer any questions post- discharge.
      • Provide a register / log of phone calls to include patient’s name and brief summary of question and visit to the COO.
    • E-Chart Dismissal Summary Duties:
      • Prepare and populate E-discharge summary,
      • Copy pertinent information from H & P to discharge summary (physical examination),
      • Add laboratory data to discharge summary,
      • Enter/update Problem List for review by PA, NP or cardiologist,
      • Provide a draft Medication Reconciliation list (Med-Rec) for final approval and signature by cardiologist, NP or PA,
      • Enter procedures e.g. heart cath reports, etc.,
      • Enter dismissal order for physician, NP) or PA signature,
      • Print patient discharge instructions and provide to patient and their family.

COMMUNICATION: Clinical Dismissal and E-Charting

    • Summarize, in person, by text, by phone results of E-charting and clinical patient dismissal plan to the discharging cardiologist, NP or PA and to, specifically, include patient-doctor directed questions which will need to be answered prior to discharge,
    • Visit with RN in-charge of the patient and communicate what questions were asked by the patient and their family, what questions were answered and if they need reinforcement or clarity by the RN in-charge of the patient.
