This position works in a team based environment in partnership with staff in the Pediatric Subspecialty Program. Provides a wide range of outpatient clinical social work services and interventions to assist individuals, families, groups and/or communities. Strives to restore, enhance, and/or protect the capacity for productive social functioning and coping with the consequences of their illness and health care status.

  1. *Accepts social work and counseling referrals from all subspecialty pediatric practices and continues to follow these patients in an outpatient setting.
  2. *Provides individual, family, and group therapy. Provides crisis management, consultation, and education services. Provides services to patients connected to the pediatric subspecialty program and throughout Montana in outreach and telemedicine settings.
  3. *Participates in the development of individual and/or group treatment plans utilizing a variety of brief psychotherapeutic, psychosocial and psychoeducational modalities.
  4. *Identifies and recognizes rapidly changing situations and quickly intervenes to prevent or resolve emotional crisis for patients and their families.
  5. *Works with medical, clinical and support staff to arrange, mobilize, utilize, and coordinate a comprehensive range of community resources that effectively meets the identified patient and family needs.
  6. *Collaborates and coordinates with inpatient social worker services for a continuity of care.
  7. *Takes a leadership role in the facilitation of interdisciplinary communication and guides team member’s interactions with patient/families experiencing complex psychosocial issues.
  8. *Formally and informally educates patient/family and health care team members regarding the social work role, pertinent patient care issues, and population specific information.
  9. *Serves as a patient advocate and liaison with physicians and other healthcare professionals.
  10. *Maintains data related to Pediatric Subspecialty utilization and outcomes.
  11. *Documents patient and family assessment, history, diagnosis, treatment and transition care plan, and progress, in the medical record.
  12. *Complies with the legal and regulatory laws, reporting, and practice standards within their scope of practice.

  1. Master’s Degree in Social Work or related counseling field from an accredited university required.
  2. Current Montana LCSW licensure required.
  3. Minimum of two (2) years’ of experience working in a healthcare setting.
  4. Ability to analyze, intervene and evaluate a range of psychosocial theory and methods in practice with clients.
  5. Possess a demonstrated knowledge of community economic resources.
  6. Demonstrated ability to synthesize and apply a broad range of psychosocial knowledge and practice interventions with a high degree of autonomy, collaboration, and skill.
  7. Possess ability to skillfully interact and intervene with patients of all ages and degrees of disability, families, physicians, other health care providers and the community.
  8. Excellent communication skills, both verbal and written, required. Possess ability to make independent judgments under stressful situations at times. Must work effectively and efficiently without direct supervision.
