Family Nurse Practitioner - KPHC Punawai

Primary Purpose:

Provide primary medical care to KPHC patients of all ages, and to participate in clinic administrative and operational decision making. Participate in teaching of professional students. The person in this position is a member of the Patient Centered Health Care Home team where each team member plays an integral role in providing patient-centered health care.

The nurse practitioner in this position will see patients at multiple clinic locations.

Essential Duties:

Elicit detailed and accurate patient histories

Assess and manage acute and chronic medical conditions

Perform physical exams, including school, sports, employment, camp and placement physicals, as well as routine well-child checks.

Order and interpret common laboratory tests and x-rays.

Record all appropriate medical information in the electronic medical record according to the Family Practice template and follow clinic procedures for documentation signage for all clinical reports.

Perform approved therapeutic procedures.

Prescribe appropriate therapeutic measures.

Instruct and advise patients regarding preventive health care measures to promote physical and mental well-being and to promote early detection, diagnosis and management of medical conditions.

Promote social as well as physical and mental well-being for children, adults and families, by way of appropriate counseling and referral to clinic and/or community resources.

Advocate for the well-being of the patient, recognizing situations where a person may be in danger of abuse and/or neglect or is not receiving appropriate care, then addressing such concerns through appropriate counseling, treatment and/or referral.

Freely consult with the staff physicians for such cases where the family nurse practitioner encounters a situation necessitating the physician's involvement or where the FNP desires the participation of the physician for confirmation of findings or of the care plan, or for continuity of care. The nurse practitioner must exhibit sound and consistent clinical judgment in recognizing those encounters that necessitate physician involvement.

Though not a legal requirement, FNP's should have their charts reviewed by a staff physician to maintain continuity and consistency of care delivery.

The FNP is to be active in the training and education of staff through day-to-day supervision of direct medical support staff and through participation in in-service training. Also, involvement in the training of community professional and undergraduate students is encouraged and supported.

The FNP is encouraged to become involved in the needs assessment of the clinic, its clients and the community it serves, either through existing clinic programs or through the development of projects and activities in this area. He or she is also encouraged to become involved in the implementation of quality assurance programs at the clinic.

Regular and reliable on-site attendance is required.

Other Duties:

Perform other related duties as assigned
  • The FNP is to participate in clinic staff meetings, team meetings, and provider meetings, as well as clinic in-services and educational presentations.
  • Observe safety and security procedures; determine appropriate actions beyond guidelines; report potentially unsafe conditions; use equipment and materials properly.
  • Attend PCMH Team Meetings
  • Attend Provider Meetings Monthly, complete Peer Reviews and Performance Evaluations according to KPHC policy and procedures.

Minimum Qualification Requirements:

Education:

Must have graduated from an accredited nurse practitioner training program.

Licensing/Certifications:

Must be licensed as an advanced practice registered nurse in Hawaii, and must meet current requirements for maintenance of licensure

Must have APRN-Rx License (Prescriptive Authority in the State of Hawaii)

Must meet CME requirements for licensure

Maintain BLS certification

Maintain CME requirements and submit professional logs annually for APRN licensure and KPHC credentialing.

Experience:

Two years of experience in community health preferred

Skills/Knowledge:

Must be eligible to participate with third party payers, with whom the clinic holds agreements and who recognize nurse practitioners as eligible participants.

Working knowledge of EMR is required

Ability to communicate effectively in a second language preferred

Working knowledge of Electronic Medical/Health Records systems

Work Hours: Clinic hours are 10 AM to 8 PM - Monday thru Sunday

Flexible schedules available, 4/10s or 5/8s

Full time or Part-time
