Klamath Health Partnership, Inc. (KHP) operates a Federally Qualified Health Center located in Klamath Falls, Oregon offering medical, dental, behavioral health & pharmacy services. KHP is seeking an experienced and innovative provider for a full-time position. The provider is committed to changing how care is delivered to meet the Patient Centered Primary Care Medical Home initiative and providing integrated services and dedicated to innovation directed at improving patient health. The Mental Health Director/ Behavioral Health Family Nurse Practitioner oversees the functioning of Mental Health Department, including the provider group. Actively supports the mission of the health center in the community. Interacts regularly with other key staff members and the Executive Leadership Team. Acts as a liaison between the providers and others involved with the clinic, as well as the broader health care community, Performs duties of Psychiatric Health Nurse Practitioner. Key Responsibilities Supervision (50%) Provides staff oversight to the Mental Health Department. Periodically reviews and updates policies and procedures as they pertain to the Mental Health Department. Reviews incident reports Coordinates clinical Quality Improvement (QI) and clinical measures. Participates in regular meetings with management staff, representing the providers and other Clinical Services staff. Participates in discussions and problem solving regarding revenue generation, budget and financial planning, strategic planning, organizational systems patient satisfaction, personnel policies, salaries and benefits, contract provisions and expansion of reduction of staff, facility and services, as these relate to mental health program concerns. Directly supervise provider group, document annual performance evaluations and other timely feedback to staff. Facilitate the preparation of the provider clinic schedule. Convene, facilitate and document regular provider meetings and oversee appropriate follow up of their content. (Record minutes and keep on file) Facilitate case management and peer review meetings. Prepare quarterly report for the Board of Directors Orient and arrange for appropriate clinical supervision and evaluation of new providers. Serve as representative of clinic at local, state, regional and national meetings by mutual agreement with the Executive Director and the provider group. Act as a liaison regarding clinical issues for the staff with Region X, US Public Health Service and National Service Corps and other groups and agencies, if appropriate. Patient Care (50%) In conjunction with primary care providers, the Psychiatric Nurse Practitioner is responsible for diagnosing, treating and managing individuals that are experiencing mental health disorders, psychosocial problems. Treating both chronic and acute emotional and/or health problems. Managing patient medications in conjunction with PCP. Conducting intake assessments of mental and behavioral conditions, providing therapy using a variety of models (CBT, ACT, DBT, etc.) and processes. Assess and evaluate “at risk” patients within a medical setting, developing prevention strategies including the involvement of additional agencies such as DHS, Tribe, SPOKES, KBBH, LCS and/or calling 911 when necessary. Provide emergency telephone counseling and support services for patients and community partners (such as the hospital.) Making effective referrals to other providers as indicated by patient’s (specialty) needs. Documentation of all patient activity including phone calls in a timely and accurate manner. Forwarding notes and information needed to other team providers such as the PCP so that they can make any needed adjustments in patient’s care. Desired Knowledge, Skills, and Abilities: A Masters of Science in Nursing degree specializing in psychiatric healthcare. Current Oregon State License Ability to initiate, adjust, monitor and recommend changes in drug therapy in conjunction with PCP Multicultural abilities are necessary, and Spanish speaking is another strong preference. Knowledge of behavioral health medicine, and use of evidence-based treatment models. Ability to make accurate clinical (DSM – 5) assessments, using a variety of assessment tools. Ability to work as an effectively team member in a high stress/paced environment. Computer skills, including experience with an EMR or other electronic records format.
