General Overview:

JSSA's Mental Health Services department has an immediate opening for a part-time (could become full-time) Care Coordinator to to join our growing team in our Fairfax office . We are a dedicated team of therapists committed to providing cutting-edge services and programs and support for children, adults, seniors and families. Our therapy services include a tailored assessment, and customized treatment and support.

Case Management Responsibilities:
  • Provide care coordination within a Systems of Care framework using a Wraparound practice approach to young people transitioning to adulthood (ages 16-24).
  • Serve as a facilitator, advocate, and liaison, working with individuals (and their support team as necessary) to identify strengths and needs.
  • Develop an individualized, comprehensive plan of care with short-term (3-6 months) goals focused on transitioning to independent adulthood.
  • Identify services and resources to support the successful implementation of interventions.
  • Facilitate meetings with individuals support teams as needed.
  • Provide service referrals and connections.
  • Identify and address gaps in services and supports which prevent the successful implementation of interventions.
  • Submit all required forms and data according to established expectations.
  • Participate in department staffing, case conferences/consultation meetings as appropriate.
  • Attend all relevant training
  • Document all work as necessary in electronic health record and other tracking tools according to established expectations

Position Qualifications:
  • Bachelor's Degree (Master's preferred)
  • Spanish speaking highly desired
  • Minimum of 1-2 years of experience performing case management (or similar) duties outlined above.
  • Minimum of 2 years experience working children and families with intensive needs.
  • Must be proficient working in virtual environment and comfortable engaging supervisory and team support in a in a fast-paced team environment.
  • Proficient computer/database skills (Microsoft Office), minimum of 1 year of experience working with an electronic health record or similar technology related to service delivery.
  • Experience in community-based consultation and outreach
  • Will consider recent graduates
