Job Description

POSITION SUMMARY:
The Psychiatric Rehabilitation Consultants are enthusiastic and passionate professionals who enjoy working with people who have severe mental illness to maximize their self-sufficiency and level of independence. PRP Consultants have flexible schedules that can serve one to ten individuals on their caseload. Services which are provided are individualized and based upon a detailed assessment and corresponding Individual Rehabilitation Plan for each individual served. The PRP Consultant provides support with accessing and coordinating benefits as well as services and community-based resources. PRP services are provided primarily off site, within the home and in the community. The individual receives assistance with developing life skills training to include organizational skills, time management skills, nutrition, budgeting etc. This job would supplement another full time or part time job for the professional who has strong desire to serve individuals with mental illness in the community.

Principle Responsibilities and Duties:

  • Design and run designated number of groups/classes. Instruct, assist, and monitor the day-to day program activities and assist clients in meeting program expectations. Focus on symptom management tasks and coping skills. Assist in maintaining good customer relations. Assess and monitor rehabilitation and treatment needs of a caseload of psychiatrically disabled clients.
  • Provide in-home rehabilitation in daily living and social skills.
  • Provide case management services such as securing and monitoring entitlements and linkages of services.
  • Provide crisis intervention as necessary.
  • Develop and write the IRP with the client and Treatment/rehabilitation team.
  • Monitor and document monthly progress and communicates this with the rest of the team.
  • Perform monthly visits to off-site consumers as specified by their treatment plans.
  • Encourage the development of natural supports—i.e. the client’s family.
  • Coordinate with other service providers and agencies involved with the clients.
  • Attend/participate in required staff meetings.
  • Orient new consumers to staff members, visitors, and volunteers of the program.
  • Other assigned duties as required.

Qualifications:

  • Professional willing to advocate for clients
  • Reliable transportation with valid driver’s license
  • Detail- oriented, organized, and can be a problem-solver
  • Undergraduates with a BA/B.S or Masters’ in a human services field preferred. Candidates with High School diploma and at least one year experience are also eligible to apply.
  • Certain programs and positions may require access to a vehicle during working hours for work related duties, including transporting clients as necessary. When access to a vehicle is required vehicle insurance must name the employee as an insured driver.
