DUTIES AND RESPONSIBILITIES
At the direction and guidance of the QI Coordinator:
• Reaches out to newly enrolled patients to welcome them to our practice and to establish initial care
• Collaborates with providers and patients to manage annual Well Visits, preventative and chronic care needs
• Conducts Electronic Health Record (EHR) audits to assess gaps in care, analyzes and identifies trends
• Performs patient outreach to educate patients about their gaps in care and makes plans to close clinical quality measures as set forth by UDS and HEDIS
• Tracks and documents patient status in respect to clinical quality measures
• Analyzes clinical quality data and assists in preparing reports and designing solutions to quality issues
• Coordinates transition of care for patients admitted to the Emergency Department by assuring patient has a follow-up appointment, receives hospital medical records and tracks frequency of acute visits
• Works independently within general direction and guidelines, yet recognizes when to seek assistance
QUALIFICATIONS (Education/Experience/Knowledge)
• Completion of approved Medical Assistant course or other equivalent healthcare education
• Minimum 2-years experience as a Medical Assistant in Primary Care
• Bilingual in English and Spanish
• Good verbal and written communication skills with patients and co-workers
• Well organized, self-motivated, independent thinker who can work well with others
• Knowledge of Electronic Health Records – Allscripts preferred
• Ability to operate a computer with the ability to effectively enter and retrieve information
• Technically inclined with analytical and critical thinking skills
• Basic Microsoft Excel, Outlook, Word skills desirable
• CPR certification desirable
• Strong ethics and a high level of personal and professional integrity

• Remote Position with occasional needs to attend meetings at Administration and/or Clinics
