Job Description

Associates Degree in Nursing field, five (5) to seven (7) years of nursing experience with RN license required; or equivalent combination of education and experience. Incumbent should be able to apply common sense understanding to carry out instructions furnished in written and oral form, possess high-quality verbal and written communication ability, strong customer service skills, and be knowledgeable of all Department of Health and Department.Must be proficient in the operation of computer applications, general office and medical equipment.

Job Summary

This position is primarily responsible for maintaining clinic services and coordinating Wellness programs for Independent Living residents and staff. Additional duties include scheduling and providing medical services to employees and residents.

Key Responsibilities

Nursing and Clinical Operations:

Operates clinic and performs routine nursing functions to maintain the highest level of patient service and satisfaction.

Assists residents as necessary with leaving messages for providers to ensure messages are followed up on appropriately.

Calls in all prescription refills as directed by the provider in a timely manner.

Assists receptionist in patient check-in and check-out duties as necessary.

Effectively performs various procedures as directed by the providers.

Performs/records quality control on in-house lab tests to meet departmental standards. Does glucometer checks.

Attends Health Center meetings with Director of Nursing to provide information on residents that may affect medical services provided and advises on condition of resident.

Resident Care

Performs a variety of services to ensure excellent resident care in the Clinic.

Accurately collects data for resident care including chief complaint, history of present illness, current medical and social problems, past medical history, current medications, allergies, and vitals; recognizes emergency procedures that need to be performed and responds appropriately. Provides emergency response.

Schedules residents for initial and follow-up appointments and ensures adequate coverage is available for smooth patient flow.

Performs all resident care duties within DOH/DSS guidelines.

Assists provider with patient referrals; displays a thorough understanding of guidelines for all appropriate referral programs.

Performs ear irrigations according to departmental protocols as ordered by the provider.

Takes vital signs of patients and records

Transfers patients from Independent Living to Health Center and contacts the family as necessary. Assess resident condition with UAI and communicates with Care Plan Team on findings.

Examines processes and or resident scheduling for lab work, transferring to acute care and clerical work

Administrative Functions

Performs clerical functions for health clinic as well as other general office duties as assigned.

Performs word processing and clerical duties as requested in a timely and accurate manner; proofs all work before forwarding to resident or staff, ensuring 100% accuracy.

Assists in the development and distribution of resident's education materials and tools for patients and their families; hands out literature as appropriate in a timely manner.

Ensures accurate and legible documentation is maintained regarding all resident encounters.

Ensures all functions performed meet all safety and federal guidelines.

Calls in prescriptions and refills as directed by the provider in a timely manner. Keeps confirmation in case of questions.

Assists provider as necessary with resident referrals and accurately completes all referral paperwork.

Ensures exam rooms are adequately stocked and prepared prior to resident hours in contributing to the maximum efficiency of the Clinic.

Schedules and notifies residents and employee of clinic appointments. Confirms appointment two (2) days prior to scheduled day.

Employee Health

Provides employee with medical services to ensure the highest quality of physical health and mental wellness.

Conducts PPD screening and physicals for new employees and annual recertification; obtains list of employees from Human Resources of due employees.

Coordinates x-rays for employees with the x-ray lab. Refers employees to Health Department if x-ray screening comes back positive for additional medical treatment.

Schedules all employee screenings and tests and communicates with employee if changes need to be made.

Maintains professional presence when representing Rock Creek; exhibits degree of professionalism consistent with standards in performance, behavior, and appearance.

Maintains resident confidentiality and privacy at all times according to policies and procedures; uses discretion when discussing confidential matters.

Demonstrates courteous and cooperative behavior with residents, families, co-workers, subordinates, supervisors, and other department staff.

Maintains positive working relationships with management and coworkers; willingly follows directions and requests as appropriate.

Exhibits professional standards through appearance and demeanor consistent personnel standards.

Demonstrates a thorough knowledge of work assignments and performs in a manner to meet quality and timeliness objectives.

Demonstrates knowledge and understanding of policies and procedures; seeks clarification or interpretation from appropriate supervisor when needed.

Uses tactful, diplomatic communication techniques in all relations with others.

Reports any injury of resident, visitor, or employee immediately to supervisor or other appropriate personnel

Provides assistance to resident, visitor, or employee in an emergency situation; following the proper emergency procedures.
