Position Summary: This key nursing position responds to patient telephone calls, electronic portal messages and walk-in patients by providing age appropriate, population specific nursing care and advice in a compassionate, caring culturally and ethnically sensitive manner. They will provide formal and informal training and mentoring to the nursing staff. They will also assist with network compliance with infection control measures, medication management and clinical quality measures.

Essential Duties and Responsibilities:

• Using symptom –based approved policies and protocols collects and assesses data/symptoms of patients to classify urgency of symptoms and advise intervention options
• Responds to patient calls and monitors provider schedules. Priority is given to red flag symptoms and same day appointment requests
• Communicates with providers and staff regarding patient needs and follows up as appropriate.
• Facilitates continuity of care
• Provides the patient with clear instructions as to course of action
• Documents patient conversation and instructions, orders or recommendations clearly and concisely in the EMR
• Monitors and manages assigned clinical inboxes
• Interacts with nursing, providers and other members of the health care team throughout the day regarding appointment scheduling and adding patients into the schedule as needed
• Uses critical thinking and listening skills and exercises clinical judgement
• Develops strategies with the Nurse Manager and Medical Director for dealing with patient-specific problems
• Collaborates with staff and other departments in the development of performance improvement opportunities
• Triages walk-in patients and assists with medical emergencies within the center
• Effectively communicates and collaborates with community agencies regarding patient care
• Responds to patient requests regarding lab/imaging results as per Network policies/protocols
• Provides guidance and mentors new and incumbent staff
• Assists with delegating or reassigning staff when requested to improve flow of center in the absence of the Nurse Manager
• Maintains compliance with standards, medication management, competencies, policies and procedures, point of care testing (CLIA), including specimen collection and proper documentation in the electronic medical record
• Attends staff meetings and participates on committees as requested
• Actively contributes to the professional development, morale and teamwork of staff while presenting a positive attitude and patient-minded vision, with patient

Qualifications: The requirements listed below are representative of the knowledge, skill and ability to perform the essential functions:

• Registered Nurse. Must possess an A.A.S. degree or higher, from an accredited school of nursing
• Minimum of three years clinical experience. Prior telephone triage or ambulatory care experience preferred
• Must possess an unrestricted license to practice nursing in New York State
• Must have a valid driver’s license and be able to travel throughout the Network. Must possess current BLS certification
• Demonstrated computer competencies including electronic medical record experience
• Knowledge of current nursing practices and standards of care
• Maintains professional and technological knowledge needed to perform job duties
• Must be proficient with rapid assessment and active listening skills
• Must be proficient with the electronic medical record
• Must be clinically competent to serve in an RN role in the Health Center setting
• Must be self-directed, detail oriented, motivated and able to work independently as well as in a team atmosphere
• Must possess excellent verbal and written communication skills in order to interact with colleagues, supervisors, medical staff and other departments
• Must be able to envision the “big picture” and how the Network’s Nursing Department activities are an extension of other Network initiatives and goals
• Standard office equipment with emphasis on computer hardware and software
• Working knowledge of medical equipment
• Occasional travel throughout the Network
• Frequent contact with individuals from many backgrounds. Moderate exposure to communicable diseases
