Hopkins Manor, Ltd

Why Work Here?

great benefits, family oriented, staff help each other

Hopkins Manor is a 200 bed skilled nursing facility located at 610 Smithfield Road in North Providence, Rhode Island. It has been family owned and operated since 1974. Hopkins Manor provides short-term and long-term care, skilled nursing services, sub-acute rehabilitation, memory impairment care including Alzheimer’s and dementia, hospice and respite services. By providing a full range of supportive nursing care services, we have the capability of easily responding to a resident’s changing needs within our home. This is a key feature that benefits our residents due to our size and our diversity of nursing care levels. For a resident, it means not having to be relocated to another facility in order to receive the quality care they need. This can make a significant difference in a person’s quality of care and quality of life. This is specially true for the frail, since abrupt relocation can be a difficult event. Over the years, Hopkins Manor has prioritized quality improvement efforts in a variety of ways. We have collaborated with the Rhode Island Quality Innovation Network - Quality Improvement Organizations (RI QIN-QIO), Healthcentric Advisors, since the 8th Scope of Work (SOW). Healthcentric Advisors “recognizes the commitment of Hopkins Manor in support of CMS’ goals to enhance health outcomes, reduce health care disparities, and achieve excellence in patient/caregiver experience.” Most recently, we have had nursing leadership changes. We have hired a new Director of Nursing, who is leading our Quality Improvement (QI) efforts and working closely with the QIN-QIO. Hopkins Manor is a member of the Healthcare Coalition of Rhode Island Response Plan to implement effective healthcare emergency preparedness and response capabilities. We are currently certified as a Center for Excellence for Wound Management by VOHRA Wound Physicians. Hopkins Manor received a Certificate of Appreciation on February 14, 2018, “for the outstanding support and excellence performance in the Research study on Infections in ‘Long-Term Care Facilities’ conducted by the Division of Infectious Diseases of Rhode Island Hospital.”
