Our Case Managers coordinate all services necessary to maximize consumer recovery and independence for children with mental illness Community based program outreach to persons served in a variety of settings. Comfortable with working with multidisciplinary teams to benefit persons served. Ensure case management compliance with agency standards and the practice model. Assist consumer to improve quality of life and maintain the highest possible ability to recover and function within the community. Implement strategies for obtaining services and supports that are goal-oriented and individualized.
Facilitate the coordination of consumer intakes, discharges, and transfers to and from other facilities programs, agencies.
Perform comprehensive psychosocial assessments to determine needs and required level and frequency of medically necessary services.
Coordinate and ensure consumer involvement in the Person Centered Planning Process (PCP), self-determination, recovery and relapse prevention. Facilitate the implementation of Individual Plan of Service.
Communicate with all services providers in Treatment Plan; coordinate to avoid duplications; monitor to ensure services are being delivered according to expected frequency and intensity.
Facilitate hospitalizations as needed and monitor progress by attending hospital medical/team meetings and coordinating with hospital social worker.
Conduct ongoing assessments of persons-served and evaluate effectiveness of Treatment goals for recovery and interventions.
Participate in legal proceedings including petitioning and testifying in clinical emergencies.
Promote a recovery centered environment and provide referral to appropriate community resources.
Educational / Talent Requirements:
Bachelor’s degree in a Human Services discipline and three years experience working with children or Master’s degree in a Human Services discipline and one year working with children is required
Licensed Bachelor’s Social Worker (LBSW) and three years experience working with children or Licensed Master Social Worker (LMSW) and one year experience working with children is preferred.
Work Experience Requirements:
Prior case management in a mental health or rehabilitation setting preferred.
Must have experience working with children.
