Physical Therapist Home Health

PRINCIPAL JOB DUTIES: - Evaluates patients for physical therapy disorders - Develops and implements appropriate treatment programs; applies therapeutic modalities effectively, designs and instructs appropriate exercise program, instructs patient in home care and preventative activities - Plans and prepares written treatment program based on evaluation of patient data - Reviews physician s referral, patient s condition, and medical records to determine physical therapy treatment required - Elicits and records information about patient s medical history - Maintains records on all patients with timely implementation of physician orders - Accounts for all chargeable times, supplies, and equipment - Cleans work area and equipment after treatment - Assists with clerical duties such as taking inventory, ordering supplies, answering phone, and taking messages - Performs other duties as assigned QUALIFICATIONS: - Graduate of an accredited school of Physical Therapy approved by the American Physical Therapy Association - Bachelors degree; masters in Physical Therapy desirable - Current California Physical Therapist license issued by State Board of Medical Examiners - Two years inpatient, outpatient and home health experience preferred - Massage Therapist certification highly desirable - Current CPR certification - Valid California drivers license with acceptable driving record; reliable automobile with minimum insurance coverage for use in the field
