Job Description

Helping Hand Nursing Services is a locally owned and operated Nurse Registry that has been providing most comprehensive medical and non-medical senior care services in Polk and Hillsborough counties for over 25 years. We are currently looking to hire an experienced RN to assist with initial client intake, client assessments, care planning, and staffing.

Job Summary

Perform new client assessments and follow up assessments in homes, facilities, and hospitals. Assist the Director of Nursing with developing care plans, staffing, and new caregiver orientation.

Job Duties:

  • Perform new client assessments and follow up assessments
  • Work with Nurse Administrator to create and update care plans
  • Update client and caregiver schedules as needed
  • Keep accurate records of all client issues and/or grievances, and promptly inform management and Administrator
  • Staff caregivers with new and existing clients as needed
  • Communicate and mentor caregivers by reviewing the care plan details, including: IADLs/ADLs to be performed, client condition, client likes and dislikes, hobbies, interests and preferences
  • Communicate with existing clients, family members, client representatives and referral sources to ensure the highest quality of services is provided
  • Participate in Friday phone calls to clients and caregivers
  • Conduct client quality calls and follow with family members, caregivers and other relevant persons for each case as needed
  • Use staffing software to document events and notes related to any and all client communication
  • Participate in the telephony process, ensuring that all caregivers are compliant
  • Promote and represent Helping Hand when communicating with referral sources and other professionals involved with clients
  • Follow proper communication processes to ensure that all updates are communicated to Client Care Coordinator and Nurse Administrator at the beginning and end of each workday
  • Maintain compliance with applicable laws, regulations, and agency policies
  • Work with integrity and follow ethical business practices
  • Performs on-call duties on an as needed

Job Requirements:

  • Passion for helping seniors
  • Well-developed customer service skills
  • Represent the organization in a positive and professional manner
  • Valid RN license
  • Valid state driver’s license and reliable automobile, current automobile insurance and willingness to operate personal car as necessitated by job
  • Excellent verbal and written communication skills
  • Proficient in the use of MS Word, Excel, Outlook, and EMR experience
  • Previous experience in Home Health/ALF/SNF or other work with seniors
  • At least 5 years of Geriatric Nursing experience
