Registered Nurse - Private Duty Case Manager

Job Description

Helms Home Care is seeking a Registered Nurse with at least 2 years experience as a Private-Duty Case Manager. Applicant must be a warm, caring person with proficient communication and computer skills. Consultation skills, active listening skills, and attention to detail in completing regulatory paperwork is critical. This part-time position will initially be a consistent part-time role until Case Management role grows into a full-time need.

Case Management Duties (Office & Remote):

- 2 nights per week on case (60 y.o. Male, Oriented, WC bound, ADLs, Hemorrhoid care, Dx: Quadriplegic)
- Development and maintenance of care plans that meet agency requirements and state regulations
- Communication with physicians and care team members regarding care plan changes
- Training, Scheduling and Supervising skilled nursing staff
- Handling client complaints and issues.
- Supply orders and management

Qualifications and Skills:
- Registered Nurse
- Minimum 2 years PDN Case Management experience
- Medicaid Case Management experience preferred
- Excellent verbal and written communication skills
- Ability to engage in active listening to uncover client needs and communicate clearly
- Willingness to work independently to deliver appropriate resources to staff and client
- Attention to detail in completing paperwork and data entry in the agency's software
- Strong organizational skills and the ability and willingness to follow through
- A sense of urgency with regards to daily tasks, responsibilities, and patient needs.

Case Management Duties (~20 hrs/week, Office & Remote):

- Training, Scheduling and Supervising skilled nursing staff
- Handling client complaints and issues.
- Supply orders and management

Registered Nurse Duties (~16 hrs/week):
- One shift per week, hands-on care with an adult male quadriplegia patient in South Charlotte
- 9p at night until 1p the following day; RN rests in the guest room from 11p to 6a each shift
- Hands-on care shift to supplement hours until Case Manager role grows into full-time needs

Qualifications and Skills:
- Registered Nurse
- Minimum 2 years PDN Case Management experience
- Medicaid Case Management experience preferred
