Job Description
Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. The Case Manager will work on a Full-Time basis out of Helio Health’s Oneida County Rapid Rehousing program.


In addition to Helio Health’s comprehensive benefits package, this position is eligible for a $1,500.00 Sign-On incentive!


Responsibilities:

  • Assist chemically addicted patients in achieving and maintaining independent housing.
  • Coordinate pre-screenings, admissions, and discharges for residents.
  • Process referrals upon receipt within seven working days and schedule interviews with prospective residents.
  • Provide services for residents admitted to the program, such as assisting resident with set up of an independent apartment.
  • Provide follow-up services at least monthly and/or dependent on resident’s needs and desires.
  • Maintain communication with treatment teams, landlords, financial sources, vendors and community liaisons.
  • Provide assistance to residents requiring financial support.
  • Maintain financial data and complete appropriate reporting forms.
  • Documents files in accurate, concise, and professional manner to meet NYS OMH requirements.
  • Provides ongoing case management services regarding housing issues to participants.
  • Provide support and assistance in crisis/problematic situations, including: informing direct supervisor and/or administrator on call of incidents, and following up to ensure completion of incident reports.
  • Ensure all documentation and entry into the electronic medical record is accurate.
  • Follow training instructions at all time when utilizing the electronic medical record system.


Qualifications:

  • Associate's degree in related field required, Bachelor's degree preferred.
  • Qualified Health Professional (QHP) preferred.
  • Two years of experience working with mental illness and/or individuals abusing/addicted to drugs.
  • Valid NYS Driver’s License preferred.
