Job Description

Full time On Call RN 7 days on/7 days off. As the On Call RN you will be responsible for providing high quality responsive care to patients and families after hours. Our regional triage team will answer all after hours calls and provide a report on the patient, as you are dispatched for an on-site visit.  You will Evaluate, conduct assessments, provide symptom management, educate the client and primary caregivers, complete care plan updates and document in our (EMR) at the time of visit.  Obtain data on current or potential physical, psychological, social and spiritual factors that may impact patient and family's needs and initiate intervention by other members of the interdisciplinary team, as appropriate.

Educational Requirements

Currently licensed as a Registered Nurse (RN) in the state and in good standing with the Board in which he/she will practice.

Position Requirements

A minimum of 6 months nursing experience within the past 3 years unless state regulations differ. Travel is necessary on a daily basis.
