We have an opening with a large Community Health Center in NM seeking a Care Coordinator Manager - Registered Nurse / RN / LCSW / LMFT / LPCC / LISW. The Care Coordinator Manager - Registered Nurse / RN / LCSW / LMFT / LPCC / LISW will be responsible for overseeing a Supervisor and a team of 12-15 FTEs who are responsible for handing care coordination for patients regarding medical, dental, and behavioral health and making outbound calls to patients. The Care Coordinator Manager - Registered Nurse / RN / LCSW / LMFT / LPCC / LISW will be responsible for the implementation, management and supervision of care coordination activities to improve health outcomes of all patients, clients, residents and communities served by the company’s clinical programs.
REQUIREMENTS FOR CARE COORDINATOR MANAGER :
- Degree from an approved school of nursing, ADN, BSN or MS; OR Master’s degree in behavioral health field.
- Minimum two years clinical experience.
- Must have one year of case management experience.
- Must have one year of management or supervisory experience.
- Knowledgeable in health care preventive services; prevention, screening and management of chronic diseases; health education techniques to support primary care services
HOURS OF THIS POSITION FOR CARE COORDINATOR MANAGER :
- Monday – Friday | 8:00 AM- 5:00 PM (NO Holidays & NO Weekends!)
