The RN Director of Case Management is responsible for coordinating the hospital Utilization Review Programs, Care Coordination, Discharge Planning, and Clinical Social Work. The RN Director of Case Management serves as a resource concerning strategies which are efficient, effective, timely, patient-centered, and support appropriate financial reimbursement. The RN Director of Case Management participates in the certification of patient stays and improving the appropriate use of resources.

Qualifications:

• Registered Nurse required
• Bachelor’s Degree in Nursing preferred
• Case Management Certification (CCM) preferred
• Director of Case Management experience in a hospital setting
• Acute Discharge Planning experience in acute care medical setting required

Responsibilities:

• Supervises concurrent and retrospective review by the Utilization Review Nurses
• Collects and trends data to facilitate patient flow
• Prepares trend sheets and other reports
• Coordinates Utilization Review Committee preparing and presenting data
• Analyzes trended information to develop action plans to improve appropriate use of hospital resources and coordinates efforts where necessary to affect changes in patterns of use
• Identifies multi-disciplinary problems and assists the responsible parties with the development of a plan/procedure for resolving them
• Monitors whether departments/committees have addressed concerns
• Advises and consults Administration and Medical Staff on Utilization issues
• Interfaces with Medical Staff to develop, support, and implement Discharge Plan of care
• Participates in Director, Concurrent Coding, Revenue Cycle, and Utilization Review Committees
• Establishes departmental policies and procedures
• Prepares and manages departmental budget
• Supervises and manages performance of caregivers within the department
• Manages Length of Stay (LOS) and DRG’s related to patient care that meets the regulatory components
