Job Description Are you a REGISTERED NURSE looking for a change with the desire to serve SENIORS in the community? We have an incredible opportunity with a Senior Services Program in Springfield, MA for a compassionate and dedicated PRIMARY NURSE (RN) ready to take on a leadership role. This reputable organization operates the largest Program of All-Inclusive Care for the Elderly (PACE) in New England. PACE helps provide older adults and their caregivers a welcome alternative to nursing home care. Participants have access to comprehensive medical services and social support while they keep the independence of living in their own homes and communities. QUALIFICATIONS ·         Valid and Unrestricted License to practice as an RN in the state of Massachusetts. ·         CPR and Alzheimer's certification or willingness to get certified within 60 days of hire. ·         Minimum 2 years of recent experience in the direct care of adults or chronically disabled persons with at least one year of caring for a frail or elderly population. ·         Reliable transportation is a must to travel between the PACE Center, participant’s homes, and assisted living facilities. ·         Case Management and coordination of care skills are critical. ·         Proven ability to work with an interdisciplinary team (IDT) model. ·         BILINGUAL-Spanish is a plus!
