Lymphedema Physical Therapist - Territory to be determined

Job Description:

Integrated Home Care is currently hiring a full time provide physical therapist. Provide physical therapy to 5-6 patients per day in their private homes. All of our clinicians use laptops to perform point of care documentation.

Duties include: -Make the initial evaluation visit and assess patient care and plans with patient/family and physician. -Teach and supervise patients and families in the home environment regarding treatment, self-care, coping and preventive health measures. -Initiate appropriate preventive and rehabilitative physical therapy. -Counsel the patient and family in meeting rehab needs. -Assist the patient with activities of daily living and facilitate their efforts towards self-sufficiency and independence. -Communicate effectively with patient, patient’s family, agency staff, physician, and other involved disciplines (i.e., social services, therapists, etc.) to carry out treatment plan objectives. -Assist in evaluating other disciplines providing services to her/his patients. -Recognize and utilize opportunities for health counseling/teaching for the patient and family in the home environment.

Education and/or Experience:

-Licensed as a Physical Therapist in MN

-Prefer previous home care experience.

-Reliable vehicle; valid driver’s license and proof of insurance

-2 years Lymphedema Experience
