Triage LPN - Must be located within our service area, but position will be remote.

An Excellent Opportunity

The triage nurse is responsible for answering calls from patients/family members and assessing their medical concern to help determine what type of care they need and consulting with case manager or providers as needed.

Job Duties and Responsibilities:

  • Communicate appropriately and tactfully with staff, patients, and family members to determine appropriate course of action. 
  • Assess the patient’s stated symptoms to determine the appropriate level of care required to safely meet the patient’s medical needs (emergent, urgent, non-urgent or home care status).
  • Utilizes standing order policy and procedures to refill e-prescriptions.
  • Works with Providers to complete and send forms for durable medical equipment, disability, medical supplies, etc.
  • Thoroughly complete documentation utilizing the EMR.
  • Attend staff development programs as required by policy and maintain current competencies.
  • Provide service in a non-judgmental, non-discriminatory manner that is sensitive to the employee's, patient's, and family's diversity, preserving their autonomy, dignity, and rights.
  • Maintain the highest standards of professional confidentiality in matters related to the patient, family, and facility staff.

 Requisites: 

  • Current, unrestricted LPN license to practice in North Carolina and/or Virginia
  • Hospice experience
