HSHSL’s Community RN’s job function is to monitor the health, safety and needs of the consumers
receiving residential services. In addition, to provide appropriate delegation and supervision of
Unlicensed Assistive Personnel (UAP) or Licensed Practical Nurse (LPN) who perform such duties as
medication administration and other nursing tasks, when applicable, and document those activities.
DUTIES AND RESPONSIBILITIES
Community RN must take the Community RN orientation training within 90 days, which is
offered though the Regional Office at least quarterly.
The RN must provide 1.25 hours per month, face-to-face monthly assessment of each consumer
within the first 1 week of month; the hours may be combined and distributed within HSHSL.
They must also be on call for 2 hours a week.
A Monthly Health Summary and Monthly review, analysis and documentation of findings based
on specified categories must be completed monthly. Payments are not rendered until all
assessments and paperwork are completed and submitted.
Community RNs are required to be current in CPR & First Aid certifications and Positive
Behavior Support training, Abuse and Neglect, and Confidentiality training. Additionally,
Community RNs must have a current Missouri RN license and background check on file.
Community RN’s monthly service log and community RN’s monthly health summary paperwork
must be completed monthly, which will include assessment, labs, review of physician orders,
review of medications, review of records, delegating nursing tasks/specialized
Community RNs are responsible for the provision of guidance and ongoing evaluation for the
delegated nursing task. The RN will be provided with a form for “Delegation of Specified
Nursing Tasks” to document non-licensed staff training, competency and delegation of specific
tasks for consumers. The form is consumer and staff-specific. It is not required for delegation of
medication administration that is covered within the scope of their medication administration
training and certification.
In the event that the consumer no longer requires the specified delegated task, the RN Delegation
for Specified Nursing Task Form can be rescinded by the Delegating RN.
HSHSL’s Community RN will be required to take vital signs and if possible a weight on
consumers every month. This will help HSHSL to intervene quickly if vitals are abnormal.
Instruct and teach a Mel Level class once or twice a month.
Complete dental forms.
Discharges of HSHSL clients from hospitals.
Delegations of duties to HSHSL employees.
Participate in consumer referrals, meet & greets, marketing and communications with the State.
Participate with correspondence in response to state documents.
Monitor and look over day program books and notes.
Other duties as apply.
***EXCELLENT GRAMMAR a must and the ability to edit documents, TYPE at least 40 WPM,
COMPUTER SKILLS, Interpersonal Skills and COMMUNICATION SKILLS.
