Heartland Hospice is searching for an experienced nurse to be a part of a growing hospice team in the Madison area. This is a full-time position mainly located in the office on a daily basis (limited if any daily travel) acting as a resource and lead for our professional interdisciplinary team, patients, families and community partners. If you have worked as an Assistant Director of Nursing, Charge Nurse, Nursing Supervisor, Quality Manager, MDS Coordinator, Team Leader or similar role and would like to grow into a hospice leadership position consider applying!
In this role you wll be responsible for the management of RN Case Managers and the support clinical team. Included in those responsibilities are providing day to day direction to the team, administering performance reviews, coaching and providing disciplinary action when needed. The role is also accountable and responsible for driving successful clinical outcomes of the team and will have a scorecard developed to measure this success. You may have responsibility for the work flow within Home Care Home Base.

Minimum of one year experience in providing medical, oncology, home health care, or hospice nursing preferred, may be specified by state. Previous management or supervisory experience preferred.
Necessary on a daily basis, local travel 25-50%.

Currently licensed as a Registered Nurse (RN) in good standing with the State Board in which he/she practices.
