Job Title: Comprehensive Community Services (CCS)-Care Coordinator
Reports To: Program Manager-CCS
FLSA Status: Full-time/Salaried

Comprehensive Community Services (CCS) is a non-traditional case management program for recovery-oriented individuals living with mental health and/or substance use disorders. CCS utilizes a person-centered, wraparound model in which the Care Coordinator is responsible for arranging and monitoring the consumer's services. The goal of CCS is to assist the consumer in reaching their life vision and encourage maximum self-sufficiency.

Essential Duties and Responsibilities:
  • Manage a caseload of individual clients - this includes completion of all CCS renewal paperwork; regular contact/home visits to monitor status and assess for needs; complete referrals to services; and other rehabilitative tasks to assist client in their recovery
  • Assist clients in building their recovery team to include formal and informal supports
  • Maintain regular contact with clients' recovery team members and monitor service progress
  • Support clients in self-advocacy and assist them in reaching the highest possible level of independent functioning
  • Conduct on-going assessment of clients to determine needs and provide and/or coordinate the delivery of
    identified services
  • Utilize a strength-based, trauma-informed, recovery-oriented, and culturally-sensitive approach and provide
    care coordination according to the CCS Core Values
  • Participate in weekly individual and group supervision sessions
  • Develop and complete treatment plans and clinical records for clients
  • Maintain all required case notes, treatment plans and other clinical records for clients according to agency
    and funder requirements
  • Other duties as assigned by the Director of Clinical Services


Qualifications:

  • Excellent communication skills both oral and written and strong interpersonal skills
  • Able to communicate, collaborate and work effectively with a diverse group of individuals - including clients, families, co-workers and other providers
  • Demonstrated cultural competency and ability to work with individuals from diverse backgrounds
  • Strong clinical writing and documentation skills
  • Skilled at clinical interviewing and person-centered approaches to case management
  • Excellent critical thinking; able to use logic and reasoning to solve problems
  • Highly developed organizational and time management skills; able to manage multiple priorities
  • Knowledge and experience in using Microsoft Office
  • Unrestricted driver's license and automobile insurance


Training and Experience:

  • Minimum of Bachelor Degree in Social Work, Psychology, or related human services field from an accredited school required; Master's Degree preferred
  • 2+ years of experience providing case-management services to adults with significant mental health and/or substance use concerns preferred
  • Excellent interpersonal skills
  • Training in Crisis Prevention and Intervention
  • Training in Motivational Interviewing
  • Computer proficient; working knowledge of Microsoft programs
  • First Aid training and CPR certification a plus
