Job Description GSP Health is looking to expand its clinical team by adding a Patient Care Coordinator to our Enid location. Patient Care Coordinators work with other medical professionals to provide high quality care to our patients. Our Patient Care Coordinator’s job is to work with staff and patients to reach healthcare goals, keep the lines of communication open, and help ensure patients understand every aspect of their care. This position is constantly evolving to help meet the needs of all patients. Requirements of the Patient Care Coordinator include: • One year of Medical experience required • Current Basic Life Support (BLS) certification is encouraged • Possess strong clinical and teaching skills and strong analytical thinking and ability to handle multiple tasks concurrently. • Strong interest in working with an interdisciplinary health team and culturally diverse patient population. • Good verbal and written communications skills. • Computer skills and proficiency in Microsoft Office. • Excellent customer service. • Compassion and empathy. Benefits • Paid Health, Vision, and Dental • Retirement Matching • Paid Time Off benefits • Company paid life insurance Responsibilities: • Consult with patients, family members, and/or caregivers to discuss their health problems. • Educate patients about their condition, medication, and give them specific instructions to reach their desired goals. • Develop a care plan to address their personal health care needs. • Consult and collaborate with other health care providers and specialists to set up patient appointments and treatment plans. • Check-in on the patient regularly and evaluate and document their progress. • Assist the care team with developing and assessing health interventions. • Attend ongoing training and courses to keep abreast of new developments in health care. • Assist with securing funding for medical care as required. • Treat patients with empathy and respect and conduct oneself in a professional manner. • Comply with organizational guidelines and health care laws and regulations.
