Clinical Case Manager

DESCRIPTION: Responsible to provide comprehensive clinical services and care management for youth and families suffering from SUD and co-occurring disorders in a residential treatment center.


  • Perform holistic psychosocial and diagnostic assessment consistent with ASAM criteria, develop collaborative treatment plan with youth and families, provide individual, group and family therapy and psychoeducational intervention. Maintain communication with families or other caregivers and other key community supports for robust aftercare planning.When indicated, perform face-to-face assessment of individuals to determine the nature and severity of SUD and/or co-occurring conditions consistent with ASAM guidelines and level of care criteria;
    • Work cooperatively with other staff as a member of the interdisciplinary team;
  • Orient participants to program goals and objectives, opportunities for growth and requirements for participation, participant rights and overall program operations;Demonstrate a person and family centered approach to treatment planning and clinical intervention;
    • Function as a liaison/advocate on behalf of participants' and their families with schools and other agencies as needed;
  • Facilitate individual, group and family counseling;
  • Conducts psychoeducation classes for participants;
  • Conduct ongoing assessment of youth and family progress to ensure that duration and type of treatment and services is appropriate to meet youth and family needs;
  • Actively work with youth, families, colleagues and community partners to ensure strong and comprehensive aftercare planning to promote continuing growth and enhanced wellness;
  • Ensure the maintenance of accurate, complete, timely and high-quality documentation and data collection to meet best practice clinical standards that comply with program policies and procedures, payor requirements and external regulatory standards;
  • Participate in case conferences with other community-based service providers;
  • Provide outreach/education/prevention services to the local community, schools, groups, and agencies as necessary and as approved by management staff;
  • Provides crisis intervention/management, generating referrals to other provider resources, as indicated, and
  • Provide care and supervision always to ensure that youth are safe, and needs are met according to developmental level, age, and emotional or behavioral needs.

* Knowledge of Relevant subject matter knowledge related to work experience in areas such as, but
not limited to, substance use, mental health, employment, and housing services
* Knowledge of and ability to follow agency mission, philosophy and policies and procedures
* Knowledge and ability to understand department policies and procedures
* Strong individual and group counseling skills
* Highly organized and detail-oriented
* Excellent verbal, written and interpersonal communication skills.

* Must be computer literate with a working knowledge of MS Word and MS Excel.

* Subject matter knowledge or experience in one or more of the following areas: substance use,mental health, employment and housing services

* Strong writing skills; competencies in writing goal based/person centered treatment plans and
progress notes as evidenced by successful completion of writing sample
* Ability to participate in CPR and First Aid training
* A working knowledge of addiction theories

* Master's degree in counseling or human services, social work or related field OR one year work
experience in substance abuse counseling or case management positions or a Master's degree in
counseling or human services, social work or related field
* Specifically trained in Recovery Coaching
* Licensed Clinician preferred

* Two (2) years of supervisory experience or three (3) years Case Management experience
* Three (3) years of work experience in substance abuse counseling or case management positions
* Minimum three (3) years of experience in case management
* Experience in working within a 12-step framework, addiction services and substance use
* Experience with youth and adolescent crisis intervention techniques
* Experience working with families affected by the disease of addiction
