WE ARE LOOKING FOR A CASE MANAGER AND AN ADMINISTRATIVE COORDINATOR AT OUR FRESH START FACILITY LOCATED IN WILMINGTON, DE!
This individual’s responsibility will be to provide case management to clients in one or more program(s). These services may include, but are not limited to: Education, Vocation, Aftercare, Medical Care, Prevention, Intake/Assessment, Economic, and Housing.
In addition, this person may handle an independent caseload and assist other personnel by ensuring proper referral of clients to all appropriate services available and necessary to them.
Duties may include, but not be limited to: providing intake/assessment services to clients, preparing reports, treatment plans and other necessary records, maintaining responsibility for the activities of one or more groups of clients, and assisting other staff members in the performance of varied work duties such as referrals and aftercare planning.
Work is performed in accordance with established regulations, policies and procedures, but the person is expected to exercise considerable initiative and independent judgment in achieving objectives.
- Bachelor’s Degree in generally recognized clinical discipline, plus specialist training for specialized population; and/or full certification as an Addictions Counselor; or Master Degree in related specialty area.
- Three (3) years of D&A counseling experience or case management.
Administrative Coordinator I:
This individual is responsible for the overall development and maintenance of the administrative function of a facility, particularly as it relates to fiscal management, personnel records, client records and files, reports and statistical data required by the Program Director, the Regional Director, Corporate Office, and monitoring/funding agencies.
Duties include but are not limited to: monitoring census, ensuring funding, the maintenance and safe keeping of client files, recording and safe-guarding client monies and valuables, preparation of reports, reporting all changes in client population to the proper persons or offices, keeping records and working with managed care agencies, funding sources, criminal justice agencies, social service agencies, etc. Duties may also include maintaining personnel records on program employees, working closely with the Director to ensure timely evaluations, reports, clearances, etc., compiling and preparing data and information for proposals and assisting in the writing of proposals, supervising clerical staff in the timely completion of their daily tasks and the management of information systems in the facility.
Work is performed with considerable independence in accordance with established regulations, policies and procedures in a coordinated effort with the Program Director and the Clinical/Program Supervisor. He/she is responsible to the Program Director for overall program effectiveness and the achievement of program goals and objectives.
- Bachelor’s Degree or equivalent credits towards Bachelor’s degree.
- Strong oral and written communication skills.
- Proficiency in Math, English, typing and Microsoft Office programs.
- Minimum three (3) years’ experience.
